Full Job Description

A Long-term care facility in Walker County, GA is looking for a Director of Social Services to join our team. We are seeking a passionate and dedicated Social Worker with the desire to make a difference in the lives of our residents, the residents’ family members, and our team members.

Responsibilities:

Assist in the planning, organization, implementation, and evaluation of the facility’s Social Services Department
Ensure social services are delivered in compliance with federal, state, and local guidelines and in alignment with the facility’s values and policies
Help develop and monitor individualized care plans for residents, addressing their social and emotional needs
Provide ongoing support to residents and families during admissions, transitions, and care conferences
Recommend appropriate equipment, services, or referrals for outpatient and home health support
Participate in policy development and recommend improvements to strengthen care delivery
Organize and participate in in-service training sessions for the social services team and other staff
Stay up-to-date on regulatory changes and professional standards related to long-term care social work
Advocate for residents and serve as a liaison among staff, families, and external agencies
Collaborate with interdisciplinary care teams to ensure holistic, person-centered care
Duties as assigned
Proficient Knowledge of Point click Care

Requirements:

Minimum of 3 years Director of Social Services experience in a Skilled Nursing or Long-term care facility
Bachelor’s degree in Social Work (BSW), or in a human services field such as sociology, psychology, special education, or rehabilitation counseling, preferred
Knowledge of federal and state long-term care regulations
Strong interpersonal, communication, and problem-solving skills
A resident-first mindset with a deep commitment to dignity, empathy, and respect
Ability to manage sensitive situations with professionalism and compassion
Must meet all applicable licensure and certification requirements in the state of practice
