Full Job Description

Job Summary:
The Case Manager assists clients in a residential environment by facilitating access to a full spectrum of services, including insurance coverage, housing, and recovery programs. The role involves providing education on addictive disorders, mental health conditions, and life skills to support clients in developing and sustaining long-term recovery plans.

Schedule:
Monday – Friday | 8:00 AM – 4:30 PM

Qualifications:

Associate degree in Human Services or related field, or equivalent experience
Bachelor’s degree in Human Services preferred
Experience as a Wellness Coach, Peer Support Specialist, or Chemical Dependency Professional candidate is a plus
Commitment to client-centered care and supporting individuals in recovery

Essential Duties:

Coordinate services for clients participating in intensive inpatient residential treatment.
Assist clients with emerging challenges and refer concerns to local community support systems as appropriate.
Conduct and maintain current needs assessments for all residential program participants.
Implement and participate in client programs, including social skills activities, community resource access, self-help skill development, parent education, and educational or substance-use lectures.
Assist clients in applying for funding sources such as DSHS or SSA assistance.
Document client progress, including chronological notes, program plans, and reviews; track vocational, educational, substance use, and employment goals, along with other client services as needed.
Consult with and represent the agency in interactions with courts and other agencies, as assigned by the supervisor.
Support program compliance with administrative directives, as well as state and federal regulations.
Collaborate with a multi-disciplinary onsite team to facilitate client transitions to the next appropriate level of care.
Mediate client disputes, ensuring conflict resolution is approached in a solution-focused manner.
Perform other duties as assigned by administrative and/or clinical supervisors.

Physical Demands:

Must be physically capable of performing the duties of the position.
Frequent use of a computer keyboard and monitor for extended periods.

Work Environment:

Moderate noise levels typical of a residential treatment facility or office setting, including computers, printers, and light foot traffic.
