Family Nurse Practitioner

Connective Wellness — Amherst, NH (on-site, with some telework flexibility)

W-2 · Growing toward full-time (if wanted) starting at ~20–32 hrs/week · $62–$70/hr

We're looking for a Family Nurse Practitioner (FNP) who's tired of the fifteen-minute visit and wants to practice medicine the way it should be done—root-cause, whole-person, and genuinely on the patient's side.

At Connective Wellness, we do healthcare differently. We specialize in functional, integrative care for people living with hypermobile Ehlers-Danlos Syndrome (hEDS), POTS, MCAS, and the complex chronic conditions that come with them. Our patients have often spent years being dismissed and misdiagnosed—we're building the opposite of that experience, and we want you to help.

What you'll do

Carry your own panel of medically complex patients and build individualized, root-cause care plans that treat the whole person.
Conduct comprehensive assessments, order and interpret labs, and prescribe within your full New Hampshire scope of practice.
Manage the layered comorbidities our patients live with—POTS/dysautonomia, MCAS, GI and autonomic involvement, chronic pain, and fatigue.
Serve as a clinical anchor in our growing diagnostic program—reviewing results, guiding treatment planning, and helping shape how we deliver care as we scale.
Fold our holistic programs and services into each care plan as confident, clinically grounded recommendations—never a hard sell.
Collaborate closely with our doctors of physical therapy, health coaches, patient advocates, and outside specialists, and help build our clinical systems as we grow.
Go as deep as you want on the piece of this world that grabs you most—dysautonomia, mast cell, pediatric hypermobility, chronic pain, the gut. Through the Prince-Cole Institute, our research and education arm, we'll back you in becoming the person other clinicians call about it.

Who You Are

A sharp clinical thinker. This is the thing we care about most. You can hold a messy multisystem picture in your head, work a differential that doesn't resolve neatly, and keep going when the answer isn't in the first three places you looked. Complex cases energize you—they don't make you want to refer out.
A Family Nurse Practitioner. FNP certification is a hard requirement, not a preference. Our patients range from kids to older adults, often in the same family, and we need a provider who can see all of them.
Deeply versed in complex conditions—or hungry to get there. hEDS and other Ehlers-Danlos types, POTS and dysautonomia, MCAS, GI and autonomic involvement, chronic pain, fatigue. If you already know this world, wonderful. If you don't yet, we will teach you—but you have to be the kind of clinician who reads the paper before the visit.
Open to functional medicine, not required to arrive with it. We practice root-cause, whole-person medicine. Formal functional or integrative training (IFM, A4M, or similar) is a nice-to-have, not a gate. We would rather hire excellent clinical reasoning and teach the functional lens than the reverse.
Local and on-site. You live in or near Amherst, NH and can work primarily in person at our clinic. We want you here, in the mix—learning the practice, knowing our patients, being a real part of the team.
Willing to hold licenses in more than one state. We're growing beyond New Hampshire. If you're already licensed elsewhere, that's a real advantage. If you're not, we pay for and manage the applications—you just have to be willing.
A true collaborator. You genuinely enjoy working across a team, sharing credit, and building something together. Ego at the door; curiosity front and center.
Built for startup life. You don't just tolerate ambiguity—you want to help build. Evolving systems, wearing a few hats, and shaping the thing as it grows sound like the fun part to you.
A clear, compassionate communicator. You can explain complicated things in plain language and make a scared, exhausted patient feel cared for.
Lived experience welcome. If you personally know what it's like to live with hEDS, POTS, MCAS, or a related condition—or to fight for someone who does—we especially want to hear from you.

What you'll need

Active, unrestricted New Hampshire APRN license, or eligibility to obtain one that is active before your start date.
FNP board certification through ANCC or AANP.
Master's or doctoral degree from an accredited Nurse Practitioner program.
Current DEA registration, or eligibility and willingness to obtain one.
Willingness to complete standard credentialing—license and education verification, background and reference checks.

Pay + perks

$62–$70/hour, W-2, commensurate with experience and credentials.
Part-time to start (~20–32 hrs/week) with a real path to full-time, benefitted employment within months as our diagnostic program grows.
Want to stay part-time for the long haul? Tell us. We'd rather build the schedule around the right clinician than lose one over hours.
$250/month healthcare stipend while you're part-time.
PTO accrues with your schedule—a full 40-hour week earns 15 days per year in years 1–2, prorated for part-time hours.
$750/year CME allowance at part-time, and CME days that don't come out of your PTO.
We pay for and manage any additional state licenses you need.
20% employee discount on all services, plus free infrared sauna and red light therapy outside of paid hours.
Charting is paid time. Documentation is built into your schedule, not something you take home at 9pm.
You'll build to a full schedule over your first 30-90 days—we don't throw new clinicians into a packed panel on day one.
No insurance billing. No prior authorizations. No coding battles, no RVU targets. We're self-pay, which means your job is the patient in front of you.
Appointments long enough to actually do the work. You will never run the same fifteen-minute visit forty times in a day.
A real team around you—DPTs, health coaches, patient advocates, and clinical support carry the parts of the care plan that aren't yours to carry alone.
Regular case conference with the team. The complicated ones, out loud, together.
Prince-Cole Institute training and certification at no cost as our programs come online.
The time and autonomy to actually practice the way you always wished you could—with a team that has your back.
