We are seeking a Housing Service Coordinator to help support the work we do.

Job Function:

The Housing Service Coordinator provides housing services for chronically homeless individuals with disabilities who are transitioning into and maintaining supportive housing. This includes creating housing plans with clients and linking them to resources. Under the supervision of the Community Health Program Manager, the Housing Coordinator works the Community Health team and other Trellus staff to ensure the full circle of services to the clients and the community.

Responsibilities:

Coordinate day-to-day supportive housing program activities and services to ensure timely, consistent, and high-quality services to clients.
Provide day-to-day supervision and oversight of assigned housing staff, including reviewing work assignments, monitoring progress, providing feedback, and identifying training and support needs.
Conduct intake assessments to determine eligibility, housing needs, and service goals for chronically homeless individuals with disabilities.
Develop and implement individualized service plans focused on housing stabilization, health improvement, and increased independence and provide intensive case management, including home visits, crisis intervention, and ongoing support to promote housing retention.
Assist with onboarding and training new staff and provide ongoing technical assistance related to supportive housing, Housing First, Harm Reduction, and program requirements.

Qualifications (Skills, Knowledge, & Experience):

Bachelor’s degree or equivalent experience in social work with experience in housing assistance.
Compassion and desire to work with diverse clients in complex life situations including housing instability.
Demonstrated ability to provide guidance, coaching, and support to staff. Experience providing lead, supervisory, or program coordination responsibilities preferred.
Ability to quickly establish rapport with clients and handle interpersonal situations professionally.
Excellent verbal and written communication skills, organizational skills, and attention to detail.
Able to drive, with valid Illinois driver’s license and body/liability insurance; or use public transportation when needed.
