Full Job Description

Case Manager

*Please note, this position requires your Illinois professional educator's license (PEL) with the special education endorsement (LBS1). This requirement is non-negotiable, and must be in place at the time of application.

Erie is built on the strong foundation of the nationally accredited program at Erie Neighborhood House, a progressive social service agency in Chicago's West Town community. Located in Chicago’s Humboldt Park neighborhood, Erie opened in September 2005 serving 80 students in kindergarten and first grade.
Currently the school has 415 students, grades Kindergarten through eighth.

BASIC FUNCTION

The Case Manager is responsible for supporting the Principal and Assistant Principal upholding the school’s mission and vision and to assist in the creation of a school environment that empowers all students to achieve ambitious academic goals, spanish language proficiency, and multicultural competence to prepare them for a diverse global society. The Case Manager will assist in the supervision of school operations related to the Special Education program and in the absence of the Principal and Assistant Principal, serves as the lead administrator consulting with the Executive Director.

JOB FUNCTIONS & RESPONSIBILITIES

School Culture & Climate
Supports Principal and Assistant Principal

● Promoting the vision/mission of the school and enlisting others to uphold this vision
● Managing the day-to-day school operations, especially related to Diverse Learners
● Collaborates with resource team to create schedule for students with disabilities in accordance with their IEPs
● Supervising students at activities, assemblies, social, and athletic events, as needed
● Supporting and promoting Erie’s school-wide approaches including Responsive Classroom,

Trauma-Informed practices, and restorative practices
● Assessing and supporting all special education programing
● Embodying and modeling professionalism, empathy, and student-centered problem solving

Instructional Leadership
● Supports student achievement and success across all contents and tiers of instruction
● Monitors and tracks trends in performance of Diverse Learners and leads data conversations with individual teachers, grade level teams, and the Diverse Learning team

● Supports coordination of required testing administration in partnership with the Assistant Principal and Technology Manager
● Supports all teachers in Tier 1 best practices to serve Diverse Learners including, but not limited to: differentiation, small group instruction, targeted interventions, accommodations, trauma-informed practices, de-escalation techniques, ABC data collection
● Ensures cohesive program implementation and collaboration of Diverse Learning team and school-wide instructional team
● Supports and consults with the MTSS team on the analysis of the T2/T3 progress monitoring data in order to decide on the next steps or the next layers of support.

Case Management

● Serves as the local school district representative at all required meetings under the IDEA and serving as the 504 Coordinator for all meetings under Section 504
● Fulfills all the Roles & Responsibilities delineated in the CPS ODLSS Procedural Manual for the local school district representative
● Ensures all IDEA and Section 504 timelines are met
● Creates systems and structures to supports Resource teachers with:
○ Ensuring that all parental notices and consents for Full and Individual Evaluations and placements are processed and provided in the native language where feasible and placed in the student temporary files
○ Sending parents/guardians copy of draft IEP materials and if applicable paraprofessional,
ESY, and Learning Environment Intervention forms, along with copies of all other conducted evaluations and any other collected data at least five school days prior to the meeting
○ Ensuring an interpreter is invited to all IEP and 504 conferences (including initial and reevaluation eligibility determination meetings, and annual reviews) for parents who are limited English proficient
○ Arranging special education and related services for a student within the school in accordance with the students IEP and contacting ODLSS to arrange assignment in another school if necessary to implement the IEP
● Coordinates all referrals for a Full and Individual Evaluation (FIE) to ensure that all required components are addressed and that the process is completed within 60 school days after receiving signed parental consent, including but not limited to, referrals for Assistive
Technology/Alternative-Augmentative Communication evaluations or functional behavioral assessments (FBAs)
● Convenes and chairs IEP and 504 conferences (including initial and reevaluation eligibility determination meetings, and annual reviews) and acts as the District Representative
● Ensures that IEP teams are utilizing qualitative and quantitative data when making service decisions in the development of an IEP
● Finalizes each student's IEP or 504 Plan in the SSM at the meeting and providing the student's parents/guardians with a copy
● Ensures that all relevant student data is entered into SSM in a timely manner

● Consults with the ODLSS District Representative regarding students who require low-incidence services or regarding problems that develop in the implementation of procedures
● In collaboration with the principal and assistant principal, oversees and ensures the implementation of all IEPs and 504 Plans for each student with disabilities in the school
● Trains and supports school staff regarding the implementation of the IDEA and Section 504 requirements
● Attends all ODLSS professional development and meetings targeted at supporting the case manager/local school district representative

QUALIFICATIONS & REQUIREMENTS

● Completion of a Bachelor's or Master's degree in special education or a Master's degree in educational administration
● A valid Professional Educator License (PEL) in one of the following areas: Special Education (PK-Age
21) endorsement or School Support Personnel endorsement with a supervisory endorsement
● Minimum of five years successful teaching experience with Elementary Special Education
● Supervisory experience preferred
● Deep experience and knowledge of Chicago Public Schools Diverse learning protocols, processes, and Illinois Special Education Law
● Expertise in Chicago Public Schools data systems like Aspen, SSM, and PowerSchool
● Bilingual (English/Spanish fluency) strongly preferred
● Experience teaching or leading in a special education or dual language setting preferred
● Familiarity with Responsive Classroom, trauma-informed practices, and restorative practices preferred
● Strong verbal, written, and interpersonal skills to support communication among school staff, community partners, parents, and other team members, required.
● Demonstrated understanding of the individual needs of students with disabilities from a variety of language and cultural backgrounds
● Strong knowledge of federal and state laws and regulations regarding the delivery of special education and related services
● Knowledge and understanding of CPS resources
● Knowledge of intervention and assessment strategies for disabled and non-disabled students
● Knowledge of multi-tiered systems of support (MTSS or RTI)
● Proven track record of academic progress of students both in the classroom and in a leadership capacity
● Thorough knowledge of child development and best practices of special education
● Passionate about urban education and demonstrated expertise in building a positive school culture
