Full Job Description

The Dovetail Project is seeking a compassionate, organized, and community-centered Fatherhood Case Manager to support fathers and families across the Chicago metropolitan area through relationship-driven case management, outreach, wellness support, and resource coordination.This role is designed to help fathers overcome barriers that may impact family stability, parenting engagement, personal wellness, and long-term success. The Fatherhood Case Manager will work directly with participants to connect them to supportive services, coordinate care, strengthen engagement, and support overall family well-being.The ideal candidate understands the importance of father engagement and is comfortable working directly within communities, homes, partner sites, and neighborhood-based settings throughout Chicago. This role requires strong relationship-building skills, professionalism, cultural responsiveness, and the ability to navigate complex support systems while maintaining consistent participant engagement.The Dovetail Project utilizes a holistic approach that recognizes the importance of mental, emotional, physical, and family wellness in supporting fathers and strengthening communities.

Key Responsibilities

Manage an active annual caseload of fathers and families participating in Dovetail Project programs and services
Conduct participant assessments, referrals, follow-up support, and resource coordination
Support fathers in navigating healthcare systems, community resources, public benefits, housing support, food access, wellness services, and related support systems
Develop individualized participant support plans focused on stability, engagement, and family wellness
Conduct field-based outreach, including home visits, community visits, partner site engagement, and participant follow-up throughout Chicago
Build positive and professional relationships with fathers while maintaining consistent participant engagement
Maintain accurate case notes, participant records, service plans, and documentation using Salesforce and related systems
Complete documentation using professional case management methods, including DAP, SOAP, GRIP, or similar documentation models
Support data collection, reporting, and participant tracking efforts using qualitative, quantitative, and mixed-method approaches
Collaborate with internal staff, healthcare providers, schools, hospitals, violence prevention organizations, and community partners to support participant care and engagement
Facilitate outreach presentations, table talks, workshops, and recruitment activities designed to engage fathers and increase awareness of Dovetail services
Represent The Dovetail Project at community events, partner meetings, outreach initiatives, and engagement activities
Utilize AI tools and digital systems to improve workflow efficiency, scheduling, documentation, communication, and participant engagement
Perform other duties as assigned

Qualifications & Skills

Bachelor’s degree required in Public Health, Social Work, or a related field
Master of Public Health (MPH) preferred
Experience in case management, community outreach, fatherhood programming, public health, family services, behavioral health, violence prevention, or related support services preferred
Strong understanding of community-based engagement and barriers impacting fathers and families
Experience working with vulnerable populations and community-centered support models preferred
Strong relationship-building, communication, and participant engagement skills
Ability to manage an active field-based caseload across the Chicago metropolitan area
Experience writing professional case notes and documentation using DAP, SOAP, GRIP, or similar case management methods preferred
Understanding of qualitative, quantitative, and mixed-method research approaches preferred
Experience utilizing Salesforce and other case management, reporting, or participant tracking systems preferred
Comfortable conducting home visits and community outreach
Strong organizational, time-management, and problem-solving skills
Demonstrated ability to utilize AI tools and technology platforms to improve productivity and workflow management
Ability to work effectively with fathers and families from diverse cultural and socioeconomic backgrounds
Bilingual or multilingual skills strongly preferred
Commitment to supporting fathers, strengthening families, and advancing community wellness
Valid driver’s license and reliable transportation required

Pay: $23.00 - $28.00 per hour
