WHO WE ARE

For almost 30 years, Signal has been managing and expanding behavioral health services in Colorado. Signal is a Behavioral Health Administrative Services Organization (BHASO) which helps ensure access to behavioral healthcare for uninsured, under-insured, and high-risk individuals by coordinating funding, supporting a provider network, and facilitating crisis response systems.

Signal's mission is to transform lives and communities by delivering high-quality and innovative professional services to behavioral health providers.

We are a non-profit organization dedicated to providing behavioral health services for youth, adults and families across Colorado. We manage mental health and substance use prevention, treatment and recovery services that are accessible, affordable, and efficacious for consumers of these services. Through effective management and coordination of services, Signal aims to increase efficiencies across behavioral health service systems, to strengthen linkages between human services systems and providers, and to more closely tailor services to the needs and strengths of consumers.

We also support Coloradoans through crisis services and crisis adjacent services for youth and families. Our youth and family service team helps to provide timely access to comprehensive clinical assessment and service linkage for youth with high acuity mental health needs.

WHAT WE ARE LOOKING FOR

The Clinical Care Coordinator III will support the care needs of those seeking or requiring treatment for behavioral health needs, particularly for priority populations such as individuals experiencing complex or high acuity mental health needs requiring high utilization of bed-based services. This role will support Signal’s commitment to ensuring the availability and access to high-quality behavioral health services in the regionally contracted counties on behalf of the Behavioral Health Administration (BHA) of the State of Colorado. The Clinical Care Coordinator III will serve as a vital member of the Clinical Services department and will help to monitor, plan, advocate, and link individuals to clinically appropriate services and resources.

A day-in-the life as our Clinical Care Coordinator III may include:

Support individuals and care teams by providing active and reflective listening as part of needs assessment and service and resource coordination.
Offer care coordination for individuals with complex behavioral health, medical, and social needs, who may require services offered by a diverse array of providers and organizations.
Engage with individuals and community care team members in the development of a care plan to address the identified needs and individualized requests for treatment and support. The care plan will be whole person centered, addressing behavioral health, physical health and social determinants of health.
Through the course of coordination efforts and care plan support, monitor and evaluate progress toward meeting treatment goals and addressing identified gaps in individual service needs.
Provide enhanced coordination of care including ongoing assessment and authorization of clinically appropriate services through use of the American Society of Addiction Medicine (ASAM) and other tools as indicated to assist in clinical recommendations.
Facilitate routine case conferences or multi-disciplinary team meetings for assigned caseload.
Plan and coordinate access to appropriate transition services between providers and levels of care, including a warm hand-off and post-discharge follow-up.
When care coordination services are no longer needed or are now being performed by a provider or alternative system of care, the Clinical Care Coordinator III will plan and facilitate discharge from care coordination services provided by Signal.
Provide timely coordination with systems partners, including but not limited to coordination with the BHA, Regional Accountable Entities (RAEs), commercial insurance, Case Management Agencies (CMAs), Department of Human Services (DHS), the judicial system, Division of Youth Services (DYS), hospitals, schools, and other care providers and systems of support.
Closely coordinate with Signal’s contracted providers to ensure quality service delivery across the continuum of care.
Participate in cross-department collaboration related to professional development, care delivery, quality monitoring, and engage in program evaluation and continuous process improvement initiatives, which may include quality review and feedback on assessment reports for Signal’s contracted assessment network to ensure consistency in meeting standards of care and producing a high-quality product.
Complete timely and consistent entry of case notes and other client information into the clinical record, including receipt and entry of any required clinical or court documentation.
Understand and adhere to best practices in care delivery and the coordination of care for priority populations as defined in the BHASO contract.
Provide in-person community-based care coordination support, as needed, to meet the needs of individuals and families served.
Support clinical department staff in their professional development by providing individual and/or group clinical supervision toward licensure, as needed.

Qualified candidates will possess:

Excellent customer service delivery and high degree of professionalism.
At least 5 years of targeted experience in performing leadership development like licensure supervision, intensive case management, quality monitoring and provision of feedback to providers, and/or clinical service delivery.
Strong written and verbal communication skills in all settings including one-on-one and in group settings.
Ability to handle confidential and sensitive information.
Solid judgment with high standards of accuracy, responsiveness, integrity and discretion.
Ability to evaluate priorities to manage several projects simultaneously, work independently and research solutions with minimal direction, and work well under deadlines and in high-pressure situations.
Ability to work in a team setting and receive and provide constructive feedback.
Organized and detail-oriented with strong time-management skills.
Proficiency in Microsoft Office (Outlook, Word, Excel, and PowerPoint) and able to learn other electronic systems.
Knowledge of mental health and substance use delivery systems including State and/or local government programs, behavioral healthcare entities, and related organizations.
Knowledge of practical and theoretical application of ASAM.
Ability to pass background check.
Ability to transport self to work and to various work assignments throughout Colorado.
Valid driver's license.

Education

Master's degree required in a relevant area such as Social Work, Counseling, Marriage and Family Therapy, Addiction Counseling, or another related field.
Independent clinical license required (LPC, LCSW, LAC, LMFT)
