Full Job Description

Overview

Join our dedicated healthcare team as a Home Health Occupational Therapist and make a meaningful difference in patients’ lives by providing expert occupational therapy services in a home-based setting. In this vital role, you will assess, develop, and implement personalized treatment plans to promote independence and improve quality of life for individuals across diverse age groups and conditions. Your expertise will support clients with neurodevelopmental disorders, disabilities, post-acute recovery, and more, ensuring they receive compassionate, evidence-based care that fosters progress and well-being.

Responsibilities

Conduct comprehensive occupational therapy assessments to evaluate patients’ functional abilities, including ADLs (Activities of Daily Living), motor skills, cognition, and sensory processing.
Develop individualized treatment plans aligned with clinical best practices, patient goals, and medical documentation standards.
Implement targeted occupational therapy programs to enhance physical rehabilitation, improve daily functioning, and support clients with autism spectrum disorders, neurodevelopmental disorders, or disabilities.
Assist patients with discharge planning by coordinating care transitions and collaborating with interdisciplinary teams to ensure continuity of care.
Support early intervention initiatives by working with pediatric clients in their homes or educational settings to foster developmental progress.
Document patient progress accurately within electronic health records (EHR) systems while maintaining compliance with healthcare regulations and medical terminology standards.
Engage in case management activities to monitor patient outcomes, adjust treatment plans as needed, and communicate effectively with clients, families, caregivers, and other healthcare professionals.

Experience

Proven experience working with individuals across the lifespan, including pediatrics (pediatric occupational therapy), geriatrics, and adult populations in home health or outpatient settings.
Demonstrated expertise in assisting with ADLs and implementing occupational therapy assessments for diverse conditions such as neurodevelopmental disorders or post-acute care recovery.
Prior experience working with students with disabilities or special education needs in school or community environments is highly valued.
Familiarity with acute care environments such as ICU or inpatient settings enhances your ability to manage complex cases requiring immediate intervention.
Knowledge of physiology and anatomy supports effective treatment plan development for physical rehabilitation occupational therapy cases.
Experience working with individuals on the autism spectrum or those requiring autism spectrum disorders support is preferred.
Strong clinical documentation skills combined with proficiency in electronic health records (EHR) management ensure accurate recordkeeping and compliance.
