Exciting Opportunity: Registered Nurse Case Manager for Home Health at BrightStar Care

Job Title: Registered Nurse (RN) Case Manager

Location: Columbus, Ohio
Schedule: Full-Time

Position Overview:
We are seeking a dedicated RN Case Manager to coordinate and oversee the delivery of nursing and personal care services for our clients with developmental disabilities, in addition to our private duty clients. The RN Case Manager will serve as the primary point of contact for patients, families, and caregivers, ensuring care plans are developed, implemented, and monitored for safety and effectiveness.

The RN Case Manager will typically be our clients’ first impression of Brightstar and they must represent the qualities of our Core Values which are:

Key Responsibilities:

Conduct comprehensive nursing assessments and develop individualized care plans.
Oversee and coordinate patient care services in accordance with state regulations, Joint Commission, and DODD requirements
Provide clinical oversight and support to LPNs, HHAs, and direct support professionals.
Serve as liaison between physicians, patients, families, and the care team.
Monitor patient progress and update care plans as needed.
Educate patients and families on health conditions, treatment plans, and best practices.
Ensure compliance with all regulatory and agency standards.
Participate in on-call rotation as needed.

Qualifications:

Current, active RN license in the state of Ohio.
Minimum of 1 year nursing experience (home health and Department of Developmental Disabilities).
Strong assessment, critical thinking, and organizational skills.
Ability to effectively communicate with our clients, their families, and their support team
Ability to work independently while collaborating with an interdisciplinary team.
Compassionate, patient-focused, and committed to high-quality care.
Reliable transportation and a valid driver’s license.
