Overview

Join our dedicated healthcare team as a Home Health Aide, where you will play a vital role in providing compassionate, personalized care to individuals in their own homes. This rewarding position offers the opportunity to make a meaningful difference in patients’ lives by assisting with daily activities, ensuring safety, and promoting independence. We are committed to supporting your growth and well-being as you deliver high-quality home healthcare services.

Responsibilities

Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
Monitor and observe patients’ health status, recording vital signs and reporting changes promptly
Administer medications and perform catheter care following medical instructions and home healthcare regulations
Support patients with meal preparation, light housekeeping, laundry, and cleaning to maintain a safe and comfortable environment
Use patient transfer methods such as Hoyer lifts or other assistive devices to ensure safe mobility
Maintain accurate medical documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
Communicate effectively with clients, family members, and healthcare teams while respecting patient confidentiality and HIPAA guidelines

Skills

Proven experience working with individuals with developmental disabilities, dementia, Alzheimer’s disease, or in geriatrics and hospice care
Strong knowledge of medical terminology, patient care procedures, and home health care assistance standards
Ability to perform patient observation, vital signs measurement, medication administration, and basic life support (BLS) or CPR certification
Familiarity with home health agency operations, home & community care practices, and home health regulations
Competence in using EMR/EHR systems for medical documentation and client communication
Physical ability to perform heavy lifting, assist with patient transfers, and provide end-of-life care when needed
Excellent caregiving skills combined with compassionate communication and attention to detail
