Full Job Description

Overview
We are seeking a dedicated and compassionate Home Health Aide (HHA) to provide exceptional assisted living to individuals with diverse healthcare needs. The ideal candidate will possess a strong commitment to patient well-being, with experience in assisting with activities of daily living (ADLs), managing medical documentation, and working within home health environments. This role offers an opportunity to make a meaningful difference in clients’ lives by delivering personalized, respectful, and professional care in a variety of settings, including assisted living, hospice, and long-term care facilities.

Responsibilities

Provide compassionate assistance with activities of daily living such as bathing, dressing, grooming, and mobility support.
Monitor and observe patients’ conditions, accurately recording vital signs and changes in health status
Assist with meal preparation, feeding, laundry, cleaning, and maintaining a safe and hygienic environment.
Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient care activities accurately and securely.
Support patients with dementia or Alzheimer’s disease through specialized memory care techniques and behavioral management strategies.
Operate medical equipment such as Hoyer lifts and ensure compliance with HIPAA regulations to protect patient privacy.
Collaborate with healthcare teams to coordinate patient care plans, including hospice or palliative services when applicable.
Engage in ongoing training related to medical terminology, infection control, and emergency response procedures such as CPR.

Qualifications

Prior experience in home health care, nursing homes, assisted living facilities, or long-term care environments is highly preferred.
Knowledge of medical terminology, medication administration, vital signs monitoring, and dementia or Alzheimer’s care.
Certification in CPR is required; additional certifications such as hospice or hospice care training are advantageous.
Experience working with individuals with developmental disabilities or disabilities is a plus.
Familiarity with EMR/EHR systems for documentation purposes.
Strong skills in patient observation, caregiving, and providing emotional support to clients of all ages.
Ability to work independently while maintaining high standards of safety, confidentiality (HIPAA compliance), and professionalism.
Valid state-specific Home Health Aide certification or equivalent credential is necessary; ongoing education is encouraged to stay current on best practices.
