Job Description

Position: *Youth Case Manager Department: Family Source Center*

FTE: 100% *Salary Range: $5,000* monthly

QUALIFICATIONS:

It is required the Incumbent for this position possess two years Intensive Case Management experience working with adults and at-risk youth. Experience in, or knowledge of community partnership building; the ability to establish rapport and relate sensitively to a multiracial and multicultural groups. The ability to work comfortably with adult & at-risk youth in both group and individual settings, preferably with experience supporting clients entering into employment and post- secondary education.

Additional requirements include the ability to handle confidential materials in a professional manner. The ability to pass an extensive background check with no history of conviction or perpetration of child abuse or physical harm to another person. The ability to work with adults and youth who have varied mental health and behavior concerns.

JOB DESCRIPTION:

This position performs Intensive Case Management services. The focus is to create an Individual Service Strategy Plan (ISSP) for adults, families and at-risk youth. Oversee and coordinate the Mental Health Subcontractors client schedule. Manage the Intensive Services Master Roster for all group and Individual Counseling Services. Advocate on behalf of the clients for LAUSD, DMH, DCFS, LA County Probation and LA County Court mandated requirements. Manage caseload of an average of 100 clients including those who demonstrate complex behavior concerns.

This position also provides ongoing support, advocacy, and guidance to participants to assist them in dealing with their personal, legal, and social service needs, especially those that interfere with their ability to meet the program requirements and their personal goals. Assure compliance with state, federal and local programmatic guidelines.

Analyze customer complaints and provide appropriate corrective actions. Assist directly or route customers to the appropriate supportive services for assistance. Ensure customer satisfaction by meeting customer needs in courteous and timely manner. Assure compliance with state, federal and local programmatic guidelines.

Additionally, this position supports, identifies and set personal development and family goals. Identify service objectives and a plan of action/goal setting with available resources needed to achieve the identified goals; and document services provided results and outcomes.
