JOB DESCRIPTION

Client Support Coordinator

GENERAL STATEMENT:

While this position specifies responsibilities, requirements and duties in certain operational areas, this person is an employee at will of Independent Living Services and will be expected to respond to such requests not specifically contained herein. This person must maintain the attitude that he/she works for the total organization, and as such, exhibit proper cooperation, enthusiasm and interest in all areas deemed necessary for the continuing success of ILS programs. Knowledge of the ILS Policy and Personnel Manuals is required.

JOB SUMMARY:

The Client Support Coordinator provides direct services to individuals enrolled at Profiles Day Program under the direct supervision of the Day Services Director. The CSC will be a part of a team of consumers, service professionals, family members, and others identified in the Individual Program Plan (IPP). The CSC will provide services including observation, supervision, training, assistance, and support in areas of need outlined in the IPP. A positive relationship with all individuals served, fellow ILS Staff, and outside organizations is vital to ensure positive outcomes. The CSC is a full time (40 hours/week), Monday-Friday daytime position. ILS reserves the right to adjust employment hours and days of operation based on requirements to fulfill contract obligations.

JOB DUTIES AND RESPONSIBILITIES:

General duties and guiding principles:

Provide advocacy when the individual's preferences, needs or talents are neglected or overlooked.
Encourage growth and recognize autonomy of individuals receiving services while being attentive reducing their risk of harm.
Develop relationships with the individuals that are respectful, based on mutual trust and maintains professional boundaries.
Help individuals I support understand and express their rights and responsibilities.
Recognize that each individual has potential for lifelong learning and growth.
Be conscious of my own values and how they influence my professional decisions
Maintain competency in my profession through continued learning and attending and being an active participant in staff meetings and trainings.
Assume responsibility and accountability for my actions and decisions.
Recognize the importance of modeling valued behaviors to co-workers, individuals receiving services and community at large.
Practice responsible work habits.
Report to work on time.
Complete all necessary documentation to assure compliance with all state and federal regulations. these may include, but are not limited to training records, personnel records, time records, incident reports, etc.
Maintain strict confidentiality for all consumer information and facility operations.

Job Duties and Responsibilities for Client Support Coordinator:

Improves client participation in the ADDT program by identifying and reducing barriers to client engagement through observation, intervention, and training.
Monitors hallways, classrooms, and common areas in person and via security camera to identify and mitigate disruptions to facilitate client success in the ADDT
Serves all consumers of the program and interacts with both direct care and admin staff in a positive and professional manner.
Participates in meetings with clients, caregivers, direct care staff, administrative staff, and outside vendors as requested by the Day Service Director. Prepares reports (verbal and written) regarding client behavior upon request.
Serves as the first point of contact of Profiles staff when additional support is needed for consumers exhibiting disruptive behaviors.
Acts as a liaison between consumers and staff when identifying breakdowns in communication.
Assists consumers with accessing solutions to their concerns in order to facilitate appropriate participation in the day program.
Completes reporting requirements per ILS and licensure policy for incidents as indicated.
Observes consumers to identify barriers to participation and works to develop effective solutions and trains other team members as indicated.
Plans and hosts monthly training sessions, workshops, meetings, etc. with clients and/or staff to maximize outcomes for the consumers participating in the ADDT program.
Makes recommendations to the care team regarding consumer classroom placement and schedule.
Maintains an updated electronic summary database that includes consumer roster with precautions and behavioral notes. Keeps updated information available at all times to Profiles staff.
Assist with ADDT staff to consumer ration if needed.
Assist with client personal care as needed.
Assist with setup and cleanup for large-group activities.
Attends relevant trainings and conferences and presents information to team upon return.

JOB QUALIFICATIONS:

Two years of experience working with individuals with developmental disabilities. A bachelor's degree in a related field may be considered in lieu of experience. This position requires initiative, self-direction, and the ability to perform assigned tasks without direct oversight.

HIRING QUALIFICATIONS:

High School Diploma or equivalent.
Valid Arkansas driver's license.
Ability to safely operate any and all vehicles, business machines, and industrial equipment necessary for the successful operation of the program.
Proof of current vehicle liability insurance.
Ability to lift 50 pounds.
Maintain current CPR and First Aid Certification.
Successfully complete a minimum of 12 hours continuing education and training in topics relative to the position annually.
Experience in areas of employment directly related to the performance of this job as included in the above "Job Duties and Responsibilities" is preferred.
