JOB DESCRIPTION
STATEMENT OF PURPOSE: The speech and Language Pathologist delivers and supervises others in the delivery of a high level of quality therapy services upon physician referral by performing evaluations and developing individual treatment plans and goals to maximize functional outcomes, actively participating in discharge planning, implementing the treatment program and reassessing treatment results. Services are delivered with the framework of a team approach, including all disciplines, significant others and the patient. These services are delivered in compliance with the facilities ethical standards, policies and procedures, national professional standards, state practice acts, where applicable, and current acceptable state of the art practice.
MAJOR TASKS, DUTIES, AND RESPONSIBILITIES:
- Supervision of Activities of the Speech Therapy Department
- May instruct and supervise the clinical portion of training for students participating in ST Programs, as well as Rehab Technicians in the rehab dept.
- Conducts on-the-job training for new employees, adapts existing training materials to meet immediate needs.
- Schedules Patients for treatment and evaluation.
- Attend department head meetings and/or sit in on required committees and maintains required records and reports needed.
- Participate in developing and updating department policies and procedures.
- Assist in maintaining inventory of materials and supplies in the Rehabilitation Department.
- Evaluations
- Review and evaluate Physician's referral and patient/resident medial records to determine Speech therapy treatment required.
- Perform evaluation of patient and complete all reports pertaining to the evaluation; plans and prepares written treatment program based on the evaluation.
- Confer with the attending physician often and other health professionals involved in the care of the patient, to obtain additional patient information, suggest revision in the treatment program and integrate patient treatments with other aspects of care.
- Diagnose and evaluate speech and language competencies of individuals, including assessment of speech, language, swallowing and aural skills as related to educational, medical, social, and psychological factors.
- Evaluate effects of treatment at various intensities and durations and adjusts treatment to achieve maximum benefits.
- Perform quarterly screens on a timely basis and refer patients as needed.
- Treatment
- Practice Speech Therapy consistent with the American Speech Therapy Association Code of Ethics and Standards of Practice.
- Maintain a patient treatment schedule consistent with the needs of the department/facility.
- Confer with the persons most closely associated with the management of the patient and keeps them informed of any additional patient needs, treatment modification and/or progress.
- Administer manual therapeutic exercises to improve or maintain muscle function; apply precise amounts of manual force and guiding patient's/resident's through patterns and degrees of movement.
- Instruct, motivate, and assist patients/residents in non-manual exercises, such as active regimes, isometric and progressive-resistive exercises, diagnose and treat patients with swallowing disorders utilizing bed side assessment techniques, as well as swallow studies performed by outside consultation.
- Record patient treatment, response and progress; consult with physician and/or nursing in the advent of adverse reaction.
- Give direct care to those residents most in need of his/her special skills and experience.
- Participate in health teaching of residents.
- Assess residents conditions and needs; assume responsibility for contributing to and activating the residents plan of care; actively involves his/her team members in the resident's plan of care.
- Keeps Head or Charge Nurse informed ofresident's conditions in a responsible and timely manner.
- Observe resident rights, e.g.: provide privacy, when in the presence of a resident include them in the conversation.
- Has a working knowledge of all the residents conditions on his/her team load; make use of reference materials to check or increase his/her knowledge of conditions of illness, and of maintenance and restorative care.
- Utilize universal precautions as prescribed by the Center for Disease Control.
- Record and Reports
- Prepare and submit all reports in a timely manner. Reports include Evaluations, Care-plans, Plan of Care, Medical Work-Up, 14-Day summaries, daily notes, weekly notes, Charge Records, Rap Keys, Screens, PPS Minutes and Projections.
- Document Speech Therapy treatment in weekly notes consistently and accurately including specific treatment rendered and response to therapy as relates to the goals.
- Assist with the development of updating goals of the patients.
- Documentation is written in proper English.
- Accurately prefects DIC dates on notes for Weekly Rehabilitation meetings.
- Maintains confidentiality of all patients' records.
- Meetings and Conferences
- Attend all departmental meetings on time.
- Attend Care-plans with residents/patients and their family, as well as, report on patients' needs and progress, discuss reports and further goals, and DIC plans.
- Attend all mandatory in-services and participates in in-servicing other departments.
- Functions Related to Personnel
- Supervise and guide non-professional personnel in work organization and administration of resident's care.
- Maintain a teaching relationship with team members by precept and example in the interest of safe and effective resident care.
- Work cooperatively as a member of the team.
- Participate in obtaining relief coverage as necessitated by staff member absences.
- Keep abreast of new techniques and trends in the field of Speech Therapy.
- Attend conventions, meetings, or special courses whenever possible and share with the department/personnel information gained as a result.
- Maintain good public relations with all community sources.
- Conduct self with professional dignity; is pleasant and tactful; understand the impo1tance of good human relations; able to motivate others; show initiative, good judgment, and enthusiasm.
- Assist with recruitment of applicants for positions, orients new personnel to the program and procedures.
- Functions Related to Policy, Procedures, and Organizational Relationships
- Assist in maintaining the facilities philosophy and in meeting its objectives.
- Functions within the established personnel policies.
- Keep superiors and subordinates informed as indicated to maintain good resident care and good morale among staff members.
- Keep current on and adhere to the department/facilities policies and procedures.
- Maintain good working relationships with all facility personnel.
- Foster and maintain cordial relationships with the residents, their families and friends.
- Perform all other duties as assigned.
QUALIFICATIONS:
- Skilled Nuring Home Facility Experience
- High school diploma, general education degree or equivalent.
- Must be a graduate of an approved School of Speech Therapy with a preferred minimum of two years clinical experience; with a valid License issued by the State of Florida.
- Knowledge of the clinical theory of Speech Therapy; Practical knowledge of Speech Therapy skills and standards, and consistent adherence to the Speech Therapy Practice Act.
- Must be able to relate professionally and positively to facility staff and work cooperatively with other associates at all levels.
Ability to communicate effectively in English through verbal and written means.
- Possess physical strength to assist residents in transferring from bed, wheelchair, etc., repeatedly during work shift.
- Possess physical stamina to remain standing, be able to stoop, bend, reach, and lift during work.
- Ability to work under stress and in emergency situations.
