What You’ll Do

As a Home Health RN, you’ll be the heart of patient care, bringing clinical expertise and compassion directly into the home. You will:

Build meaningful relationships with patients and families
Create personalized care plans that truly make an impact
Coordinate care as a trusted Case Manager
Educate and empower patients and caregivers with confidence
Collaborate with an interdisciplinary team that shares your commitment to excellence

What We’re Looking For

Currently licensed as a Registered Nurse (RN) in the State of Colorado
3 years of nursing experience preferred (Home Health or Med-Surg a plus)
Strong clinical skills and a passion for patient-centered care
Current CPR certification
Valid driver’s license, auto insurance, and reliable transportation

Salary: Per Visit Schedule

PASCO/SW later split into an independent agency in 2006 and in 2021, became part of the Cornerstone Inc. family.
