Full Job Description

Job Overview

The Physical Therapy Patient Care Coordinator/ Physical Therapy Tech serves as the central point of coordination for scheduling, patient flow, and overall organization within the Physical Therapy department. This role ensures that patients move efficiently from referral through discharge while supporting therapists, assisting patients, and maintaining a structured, high-performing clinic environment.

The primary objective of this position is to ensure that every patient receives timely, consistent care and that therapist schedules are optimized to support both patient outcomes and clinic productivity.

Responsibilities

Scheduling

Schedule all new patient evaluations in a timely manner.

Schedule follow-up visits based on therapist recommendations and plan of care.

Schedule required re-evaluations and progress notes with the Physical Therapist every 30 days. Ensure that treatments between re-evaluations are appropriately scheduled with the Physical Therapist Assistant (PTA).

Ensure patients remain on track to complete their prescribed treatment plans.

Maintain therapist schedules at optimal capacity.

Proactively fill cancellations and open appointment slots.

Maintain and actively manage a cancellation/wait list.

Patient Care Coordination

Track each patient throughout their episode of care by monitoring:

New referrals from physicians within the orthopedic practice

Insurance authorization status

Evaluation scheduling and completion

Timely scheduling of 30-day re-evaluations/progress notes with the Physical Therapist

Active treatment plans and PTA scheduling between re-evaluations

Remaining authorized visits

Pending discharges

Inactive patients requiring follow-up

Communicate regularly with therapists regarding scheduling needs, patient progress, re-evaluation timing, and any authorization concerns

Physical Therapy Technician Responsibilities As clinic flow allows:

Prepare treatment areas for patient care.

Clean and sanitize equipment according to clinic standards.

Organize and maintain supplies.

Assist therapists with patient setup and exercises within scope and clinic policy. Help maintain a safe, clean, and welcoming treatment environment.

Front Desk Responsibilities

Answer incoming phone calls in a professional and courteous manner.

Greet patients and manage check-in and check-out processes.

Assist patients with required paperwork and documentation.

Collect necessary forms, signatures, and information.

Support general front office operations with regards to physical therapy clinic as needed.
