Job Overview
We are seeking a dynamic and detail-oriented Hospice and Home Health Intake Coordinator to join our healthcare team. In this vital role, you will serve as the first point of contact for patients seeking home health services, ensuring a smooth and efficient intake process. Your energetic approach will help facilitate seamless communication between patients, healthcare providers, and insurance companies, contributing to exceptional patient care and operational excellence. This position offers an exciting opportunity to make a meaningful impact in the home health setting by coordinating care plans, verifying insurance coverage, and managing medical documentation with precision and professionalism.

Duties

Act as the primary liaison for new patient intake, gathering comprehensive medical histories and verifying insurance coverage including Medicare and Medicaid
Review and process all incoming documentation such as referral forms, physician orders, and medical records using EMR (Electronic Medical Records) systems like Kinnser Wellsky
Coordinate with clinical staff to schedule initial assessments and ensure timely patient onboarding
Verify insurance benefits, conduct insurance verification, and handle prior authorizations to facilitate smooth service delivery
Maintain accurate documentation review and ensure compliance with HIPAA regulations for patient privacy and data security
Utilize computer skills to update patient records in various EHR (Electronic Health Record) systems such as Kinnser Wellsky
Communicate effectively via multi-line phone systems while demonstrating excellent phone etiquette and customer service skills
Support administrative tasks including filing, data entry (10 key typing), appointment scheduling, and managing medical office records
Collaborate with billing teams to assist with medical billing processes, CPT coding, ICD-10/ICD-9 coding, and documentation review for accurate claims submission
Assist in creating detailed care plans by reviewing clinical documentation and coordinating with healthcare providers

Requirements

Familiarity with EMR/EHR systems such as Kinnser Wellsky is highly preferred
Knowledge of medical terminology, ICD-10/ICD-9 coding standards, CPT coding, and Medicare guidelines is essential
Excellent organizational skills with the ability to multitask efficiently in a fast-paced setting
Strong communication skills with a professional phone manner and customer service orientation
Bilingual abilities are a plus to effectively serve diverse patient populations
Proficiency in Microsoft Office applications including Word, Excel, and Outlook; familiarity with Eaglesoft or similar dental management software is advantageous
Previous clinic or hospital experience involving intake coordination or front desk duties is preferred
Ability to handle sensitive information discreetly while maintaining accuracy in documentation review and data entry
