Full Job Description

Job Summary

We are seeking compassionate and dedicated Home Health Aide (HHA) / Certified Nursing Assistant (CNA) professionals to provide exceptional in-home and community-based care to a diverse range of clients. In this vital role, you will deliver personalized support that promotes independence, dignity, and well-being for individuals across various stages of life, including seniors, individuals with disabilities, and those receiving hospice or end-of-life care. Your energetic approach and attention to detail will help create a safe, comfortable environment where patients thrive.

Responsibilities

Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility assistance using proper patient transfer methods and equipment such as Hoyer lifts.
Monitor patient health by observing vital signs, recognizing changes in condition, and reporting findings accurately in electronic health record (EHR) or medical documentation systems.
Administer medications following home healthcare regulations and medical orders while maintaining strict patient confidentiality in compliance with HIPAA standards.
Provide specialized care for patients with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies.
Support patients during end-of-life or hospice care by offering compassionate emotional support and ensuring comfort measures are met.
Perform light housekeeping duties such as cleaning, laundry, meal preparation, and maintaining a safe environment to promote overall well-being.
Document all patient interactions thoroughly in medical records and communicate effectively with clients and their families to ensure clarity of care plans.

Requirements

Valid HHA or CNA certification with current licensure as required by state laws.
Prior experience working in assisted living facilities, nursing homes, or home healthcare settings is highly preferred.
Demonstrated skills in patient observation, medical terminology, catheter care, and taking vital signs.
Knowledge of home health care assistance protocols, including medication administration and basic life support (BLS) certification such as CPR.
Experience working with individuals with developmental disabilities or geriatrics is advantageous.
Ability to lift heavy objects safely and perform physical tasks involved in patient transfer methods using equipment like Hoyer lifts.
Familiarity with EMR/EHR systems for accurate documentation; knowledge of home & community care regulations is a plus.
Strong communication skills to foster positive client relationships and team collaboration.
