Responsibilities

Perform thorough patient assessments, including vital signs, physical examinations, and medical history documentation using electronic health record (EHR) systems such as PCC- Point Click Care.
Administer medications and injections accurately, including IV infusions, medication management, and sterile technique adherence.
Provide specialized care for patients with complex needs such as skilled long-term care residents, memory care patients, or individuals requiring hospice and palliative medicine services.
Assist with diagnostic evaluations, specimen collection, laboratory procedures, and interpreting test results like ICD-10 coding or ICD-9 when applicable.
Manage acute pain through appropriate interventions, including wound care.
Support patient recovery through procedures such as catheterization, tube feeding management, ventilator support, and airway management.
Ensure compliance with HIPAA regulations and maintain meticulous medical documentation for legal and quality assurance purposes.
Collaborate with physicians and other healthcare professionals in medicine or specialty areas like infectious disease care.
Participate in discharge planning, case management activities, and health coaching to promote optimal patient outcomes across inpatient and outpatient settings.

Experience

Demonstrated proficiency with EMR/EHR systems such as Point Click Care for medical records management.
Experience providing specialized nursing services such as dialysis support, ostomy care, infectious disease management, or behavioral health interventions.
Skilled in handling complex patient populations including geriatrics in long-term care facilities or assisted living environments; experience working with individuals with disabilities is highly valued.
Competence in performing laboratory procedures like phlebotomy or specimen processing; familiarity with laboratory experience is advantageous.
Ability to work effectively across multiple disciplines while maintaining high standards of aseptic technique and infection control protocols.
