Job Overview
We are seeking a dedicated and compassionate Home Health Aide (HHA) to join our caregiving team. The ideal candidate will have a strong background in patient care, with experience in assisted living, nursing homes, or home health environments. This role involves providing essential support to individuals with diverse needs, including those with developmental disabilities, dementia, or recovering from illness, ensuring their safety, comfort, and dignity.

Responsibilities

Observe and monitor patients’ health status, including vital signs and overall condition.
Assist with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility assistance.
Administer medications accurately following medical documentation protocols while maintaining HIPAA compliance.
Support patients with memory care needs, including Alzheimer’s and dementia care strategies.
Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely.
Maintain detailed medical records using Electronic Health Record (EHR) systems and ensure proper documentation of patient care activities.
Provide home and community-based care services including meal preparation, light cleaning, laundry, and companionship to promote a safe and comfortable environment.

Experience

Prior experience working in home health, nursing homes, assisted living facilities, or long-term care environments.
Knowledge of medical terminology relevant to patient care and medication administration.
Experience working with individuals with disabilities or developmental challenges.
Certification in CPR is required; additional certifications such as hospice or Alzheimer’s care are preferred.
Familiarity with EMR (Electronic Medical Records) systems and medical documentation practices.
Demonstrated ability to perform patient observation accurately and respond appropriately to changing health conditions.
