Full Job Description

Practice Type: Home Visit / House Call Medicine — Chronically Ill & Homebound Patients

Location: Plano, TX, and surrounding DFW service area (field-based, in patients' homes)

Job Type: Full-Time or Part-Time (DOE) — flexible route scheduling

Compensation: Fee-per-visit: $125 per in-home visit; $65 per telemedicine visit. Estimated annual income for

full-time physicians: approximately $276,800/year (based on 8 home visits/day, 5 days/week, 46 working

weeks/year, plus 60 telemedicine visits/month).

Key Benefits: Medical malpractice insurance fully covered by the practice; dedicated driver provided for all

home visits — no personal vehicle required

About the Role

We are seeking a compassionate, experienced Physician (MD or DO) to provide in-home medical care to

chronically ill and homebound patients in the Plano area. Our patient population frequently includes individuals

managing complex, multiple chronic conditions, and mental illness is often present alongside physical health

needs — comfort and skill in addressing both is essential. A dedicated driver is provided for every visit, so your

time in the field is spent entirely on patient care, not transportation or parking.

Responsibilities

Conduct comprehensive in-home medical visits for chronically ill and homebound patients, including

diagnosis, treatment, and ongoing chronic disease management

Screen for and manage co-occurring mental health conditions (e.g., depression, anxiety, cognitive decline,

dementia-related behavioral symptoms), coordinating with behavioral health or psychiatry as needed

Develop and update individualized care plans in collaboration with family caregivers, home health agencies,

and other care team members

Order and review labs, imaging, durable medical equipment, and specialist referrals as needed
Prescribe and manage medications, including reconciliation for patients on complex regimens
Maintain timely, accurate documentation in the EHR, including home-visit E/M coding (CPT 99341–99350)
Communicate visit summaries and care plan updates to patients' families and caregivers
Participate in care coordination with the practice's general practice and billing teams

Qualifications

MD or DO degree from an accredited medical school
Active, unrestricted Texas medical license (or eligibility to obtain)
Board certification or board eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
Active DEA registration
Experience or strong interest in home-based primary care, geriatrics, hospice/palliative care, or complex

chronic disease management

Demonstrated comfort and competence managing patients with co-occurring mental illness
Strong communication skills for coordinating with patients, family caregivers, and multidisciplinary care

teams

Valid driver's license preferred (not required to drive on the job — transportation is provided)

Compensation & Benefits

$125 per completed in-home patient visit
$65 per completed telemedicine visit
Estimated full-time annual income: ~$276,800/year, based on 8 home visits/day (5 days/week, 46 working

weeks/year) plus 60 telemedicine visits/month

Compensation reflects visits personally performed by the physician; no cap on visit volume
Medical malpractice insurance fully covered by the practice
Dedicated driver provided for all home visits
Health, dental, and vision insurance considered after 6 months of engagement
2 weeks paid time off after 6 months of engagement
CME allowance of $1,000 after 6 months of engagement
Retirement plan offered after 6 months of engagement
Engagement type: 1099 independent contractor

Work Arrangement

This is a field-based role visiting patients in their homes throughout the Plano/DFW area on a scheduled daily

route. A practice-provided driver handles all transportation between visits, allowing you to focus fully on clinical

care.

Pay: $150,000.00 - $250,000.00 per year
