Full Job Description

We are seeking a committed and dedicated Caregiver Support Coordinator who serves as a servant leader with an understanding of the aging industry serving the elderly. Provide administrative support for the Caregiver Support Program. Assist and coordinate planning of program outreach initiatives. Coordinate and assist in referral process. Collect initial intake assessment information from seniors and caregivers seeking services and interview clients to determine needs via phone, e-mail, online, and in person. Coordinate respite and companionship services for constituents.

Essential Duties and Responsibilities

Conduct comprehensive TCARE assessments to identify senior/caregiver needs

Determine need for counseling, support groups, training, financial assistance, respite, or other services

Link caregivers to available support program services and community resources

Provide on-going support via telephone with caregivers who are receiving services to identify service delivery problems, to validate the current plan of t, and to note changes in condition or needs

Collaborates with Caregiver Services Program Manager on coordinating volunteer-based services, evidence and non-evidence training, and special events.

Participate when appropriate with senior/caregiver conferences through presentations and information exchange

Problem solves with providers and consumers; handle grievance resolutions

Utilize agency resource database and other available community information to provide seniors and caregivers with the most accurate and up to date referral information

Development and identification of caregiver resources and community partnerships

Re-assessment if warranted by change in health status

Tracks and identifies problems through caregiver evaluations and make recommendations for improvement via follow-ups.

Maintain an effective record keeping system and confidentiality of caregiver intake, assessment and consumer data

Handle consumer data in accordance with HIPPAA

Prepare and submit required reports within established time frames

Prepare documentation and assist in annual program evaluations as needed

Promote the program to potential caregivers, support groups and the public through outreach efforts

Maintain up to date knowledge of eligibility for community programs

Able to work some weekends and evenings

Adhere to established budget

Skilled in using organizational, communication and interpersonal skills on a one-to-one basis, as well as in small and large group settings

Skilled in Person-Centered planning and decision-making support counseling

Strong verbal, written, and listening communication skills

Identify and search for caregiver resources

Ability to develop reports and assessments as necessary

Ability to travel to clients' homes in designated Region 1-A area (drivers' licensure and reliable vehicle required)

Ability to travel to local, state, and national meetings, trainings, and conferences (when applicable)

Complete training required for position

Ability to collaborate well with colleagues and work independently as needed

Excellent customer service and organizational skills with attention to details required

Competent in computer applications including Windows 2010 and Microsoft Office 2010 software applications

Excellent time management skills

Knowledge of DAAA rules and regulations in reference to HIPAA Compliance.

Performs other duties as assigned

Education:

Minimum associate degree (Bachelors preferred) in the Human Services field.

Required:

Ability to travel to clients' homes in designated Region 1-A area (drivers' licensure and reliable vehicle required)
Strong verbal, written, and listening communication skills
Ability to travel to local, state, and national meetings, trainings, and conferences (when applicable)
Able to work some weekends and evenings
Excellent time management and detail oriented

Experience:

Experience in community outreach in the aging network preferred.
Experience with Person-Centered Planning and decision-making support.
Knowledge and understanding of services for seniors and people with disabilities preferred.
Excellent communication and customer service skills.
Competent in computer applications including Windows 2010 and Microsoft Office 2010 software applications (and data entry)
