Case Management Generalist / FloaterJob Description

The Case Management Generalist / Floater provides flexible case management support across departments and program areas based on agency needs. This position assists with outreach, intake, screening, eligibility documentation, client support, referrals, HMIS documentation, and case management services for economically disadvantaged individuals working toward improved socio-economic stability.

This role is designed to provide coverage and support where needed to ensure continuity of services, timely documentation, consistent client engagement, and effective program operations. The Case Management Generalist / Floater may be assigned to various departments or programs and is expected to adapt to changing service needs while maintaining professionalism, confidentiality, and client-centered care.

Primary Duties and Responsibilities

Provide general case management support across departments and program areas as assigned.
Assist with outreach, intake, screening, and assessment services for individuals seeking program support.
Review and document financial and program eligibility in accordance with agency and funding requirements.
Maintain a caseload of approximately 5–10 economically disadvantaged individuals, depending on department needs and program assignment.
Assist in the development and monitoring of individualized case management plans, including client goals, service needs, referrals, and progress toward self-sufficiency.
Monitor and support client progress through regular contact, follow-up, and documentation.
Provide clients with guidance in basic living skills, including budgeting, comparative shopping, health and hygiene, nutrition, parenting, self-esteem building, communication, group participation, and other areas as needed.
Complete accurate and timely documentation of client intake, assessments, eligibility, case notes, goal planning, client progress, referrals, and contacts through the HMIS system.
Maintain accurate client files in accordance with agency standards, program requirements, and confidentiality guidelines.
Periodically run HMIS reports as needed or assigned.
Assist case management staff with referrals, follow-up, documentation, client communication, and file maintenance.
Provide support to case management staff and assist with monitoring service delivery when assigned.
Develop and maintain a working knowledge of community resources and social service providers.
Refer clients to appropriate community resources and provide advocacy and support as needed.
Share resource information with staff to support effective service coordination.
Identify, document, and report systemic barriers that impact client self-sufficiency.
Participate in problem-solving and brainstorming efforts to address client barriers and improve service delivery.
Provide temporary coverage or back-up support for staff, departments, and programs as needed.
Perform other duties as assigned.

Position Objectives

Help maintain continuity of services during staff vacancies, absences, increased program demand, or departmental transitions.

Support accurate documentation, program compliance, and client file maintenance.

Assist clients in working toward stability, independence, and improved socio-economic outcomes.

Strengthen coordination between departments, case management staff, and community service providers.

Contribute to a professional, organized, and client-centered service environment.

Qualifications

This position requires a minimum of a Bachelor’s Degree from an accredited college or university. A degree in Social Work, Human Services, Psychology, Sociology, Counseling, or a related field is preferred.

A minimum of three years of progressively responsible experience in the human services field is preferred.

The ideal candidate must demonstrate strong organizational skills, flexibility, and the ability to manage multiple priorities across different departments or program areas. This position requires strong case management skills, sound judgment, attention to detail, and the ability to work independently and as part of a team.

The candidate must have excellent written and verbal communication skills, strong interpersonal skills, and the ability to work effectively with individuals from diverse cultural, economic, and social backgrounds. Experience with HMIS or other client documentation systems is strongly preferred.

Physical Demands

The physical demands described below are representative of those required to successfully perform the essential functions of this position. While performing the duties of this role, the employee is regularly required to talk, hear, stand, walk, maneuver stairs, sit, stoop, kneel, crouch, and crawl.

The employee must occasionally lift and/or move up to 15 pounds. Specific vision abilities required for this position include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.
