Job Summary:

We are seeking a skilled and caring Licensed Vocational Nurse (LVN) to join our team on a Full Time basis. The Licensed Vocational Nurse (LVN) provides direct patient care in the home setting under the supervision of a Registered Nurse (RN) or physician. This role focuses on supporting patients with chronic conditions or end-of-life care by delivering compassionate, skilled nursing services and reinforcing care plans to promote comfort, safety, and quality of life.

Responsibilities:

Provide direct patient care, including vital signs monitoring, wound care, catheter care, and medication administration as permitted by state regulations
Assist in implementing and following individualized care plans developed by the RN
Observe, document, and report patient condition changes to the supervising RN or physician
Support hospice patients with comfort-focused care and symptom management
Educate patients and families on basic care needs, medications, and safety measures
Maintain accurate, timely clinical documentation in compliance with agency and regulatory standards
Assist patients with activities of daily living (ADLs) as needed
Ensure infection control and safety protocols are followed
Participate in team meetings and collaborate with interdisciplinary care team members
Travel to patient homes within assigned service area

Qualifications:

Current Texas LVN license in good standing.

Current CPR certification.

Reliable transportation and valid driver’s license.

Strong communication and organizational skills.

Ability to work independently and adapt to changing patient needs.
