Position Overview
As an RN Case Manager, you will guide patients and families through every stage of the care continuum while coordinating safe, effective, and patient-centered discharge planning. You will collaborate with physicians, nurses, social workers, rehab teams, and hospital leadership to improve patient outcomes, reduce readmissions, and ensure smooth transitions of care.

RN Case Manager Responsibilities
- Coordinate patient care across the continuum from admission through discharge
- Develop individualized treatment and discharge plans
- Perform psychosocial assessments and discharge evaluations
- Educate patients, families, and caregivers regarding treatment plans and recovery goals
- Explain insurance benefits, Medicare guidelines, patient rights, and rehabilitation philosophy
- Participate in family conferences and multidisciplinary care discussions
- Collaborate with physicians and clinical teams to ensure safe patient discharge planning
- Advocate for quality outcomes and patient safety initiatives
- Maintain accurate case management documentation and compliance standards
- Support utilization management and resource coordination
- Communicate effectively with leadership, providers, and interdisciplinary teams
RN Case Manager Qualifications
Required:
- Active Registered Nurse (RN) License
- Associate Degree in Nursing or Nursing Certificate Program
- Minimum 2 years of recent acute care experience within the last 3 years
- Experience in one or more of the following:
- ICU
- CCU
- Med-Surg
- LTAC
- Hospital Case Management
- Strong critical thinking and communication skills
- Ability to advocate for safe patient discharge planning
- Excellent organization and multitasking abilities
Preferred:
- Prior utilization review or discharge planning experience
- Experience working in interdisciplinary healthcare teams
- Knowledge of Medicare, insurance authorization, and care coordination processes
- Leadership potential and career growth mindset
Compensation & Benefits
Compensation:
- Competitive hourly pay based on experience
- Salary Range: $34.25/hour - $47.10/hour (Depends on years of Experience)
Sign-On Bonus:
- $10,000 Sign-On Bonus
- Paid in installments over a 2-year commitment
