Bilingual Care Coordinator / Scheduler – Stamford CT

EZ Care Staffing LLC, A home health care agency in Stamford is seeking an experienced Home Health Care Coordinator / Scheduler who is Bilingual, professional, enthusiastic & motivated to join team in Stamford CT.

Job Description

As a Home Health Care Coordinator your core responsibilities will include but not limited to the following.

- Working under the direct supervision of the Director of Operations to schedule agency’s caregivers accordingly with our clients.

- Communicate with clients/ client’s responsible parties & caregivers.

- Ensures that new referrals for home care services are filled promptly with an appropriate caregiver.

- Must be capable of assigning caregivers to a case based on the location of the case and the caregiver’s location.

- Review the care plan with Client’s and caregivers.

- Ensure that schedules are complete and accurate both in axis care and Sandata.

- Maintain a weekly schedule for all active clients/caregivers and document any schedule changes.

- Report any clients and caregiver issues, maintain and document communication with the team.

- Assist with last minute calls for care by finding appropriate coverage.

- Listen to the needs of clients and match them to the appropriate caregiver(s).

- Report performance issues of caregivers.

- Assist with maintaining clients and caregivers’ files.

- Follow up immediately on any discrepancies.

- Review Sandata daily to ensure that all caregivers are working as scheduled and are clocking in and out.

- Maintain accurate schedules clock in/ clock out as required to ensure accurate billing and payroll function.

Requirements/Skills:

Highly organized, self-motivated, positive attitude
Bilingual - fluent in English & Spanish
Must possess a passion for working with the aging community
Must be energetic, friendly and a team player
Must be able to prioritize workflow
Proficient in Microsoft Office
Exceptional attention to detail
Excellent customer service skills

· Excellent communication skills, both verbal and written.

· Problem-solving skills

Ability to meet necessary deadlines in a timely manner

· Experience in previous home care setting with care coordinating / scheduling

· Experience using Sandata & Axis care (Preferred)

Ability to work in a fast-paced home health care environment

· Ability to work under pressures and be able to multitask

Ability to communicate effectively with employee, clients, and their families

· Flexible - Ability to work after hours, and some weekends when needed

· Ability to do on-calls (nights and/or weekends)

· Adhere to all company policies, procedures, and business ethics

Education/Experience Requirements:

Associate degree / High school diploma or equivalent (Preferred)

Our Home Care Coordinator is responsible for the maintenance of ongoing communication with referral and intake sources for all patients. The Care Coordinator shall plan for home healthcare service coverage. The Home Care Coordinator is responsible for management of current schedules for home care workers, while striving to ensure that clients/families are satisfied and receiving excellent customer service.
