Full Job Description

Position: Community Supports (CS) Housing Navigator

Classification: Full-Time, non-exempt

Supervised by: CS Housing Supervisor

Guided by our mission to assure the safety and resiliency of children by strengthening families and their communities, Home Start, Inc., a non-profit child abuse prevention, and treatment agency has strengthened and developed San Diego’s families and communities since 1972.

Job Summary:

Provides Housing Transition Navigation, Housing Deposit, and Housing Tenancy and Sustaining Services to eligible Medi-Cal members who are experiencing homelessness or are at risk of homelessness. Partners with members, landlords, health plans, and community providers to obtain and maintain safe, stable housing while ensuring documentation, service authorization, and regulatory compliance.

Essential Duties and Responsibilities

Manage an active caseload of no less than 35 members.
Complete member assessments and reassessments every six months or as required by the Managed Care Plan.
Provide housing transition navigation, including housing search, applications, reasonable accommodation requests, and landlord engagement.
Coordinate and facilitate housing deposits and onetime housingrelated financial assistance in accordance with program guidelines.
Deliver tenancy sustaining services including landlord mediation, budgeting support, recertifications, and linkage to community resources.
Assist members in obtaining benefits and other financial resources that support housing stability.
Maintain accurate, timely documentation in required systems and complete all required tracking activities.
Conduct internal and external outreach to build relationships and develop referral partnerships.
Attend required monthly workshops, trainings, meetings, and case consultations.
Maintain confidentiality and comply with all MediCal, HIPAA, and organizational requirements.

Minimum Qualifications

Bachelor's degree in Social Work, Human Services, Psychology, or related field preferred. Relevant experience may substitute.
Minimum two years of resource navigation experience is strongly preferred. Experience in case management, housing navigation, homeless services, or communitybased service delivery is also preferred.
Knowledge of affordable housing resources, benefits systems, and traumainformed practices.
Excellent communication, organization, and documentation skills.
Valid California driver's license, reliable transportation, and insurability.

Successfully complete all required background, registry, and exclusion screenings, including LEIE/OIG checks.
