Full Job Description

Overview
We are seeking compassionate and dedicated Resident Care Coordinator to provide essential support to individuals in need of assistance with daily living activities. The ideal candidate will have a passion for helping others and the ability to create a safe and nurturing environment for clients, whether in assisted living facilities, home care settings, or group homes. This role is crucial in enhancing the quality of life for seniors and individuals with disabilities.

Responsibilities

Assess the health condition and health care needs of the resident before admission.
Serve as liaison between facility, applicant’s family and appropriate medical personnel. (Assist facility to problemsolve when change in resident’s health status occurs.)
Coordinate with other community health care professionals, such as the home health agency, hospice, physician’s offices, etc. (to acquire use of their expertise when necessary.)
Actively participate in the development of resident service plan. Participate in service plan conferences, as needed.
Establish a medication & treatment administration program before admission. Appropriate medication documentation and administration system for each resident. Monitor all medications and prescriptions.

Requirements

Previous experience in caregiving, senior care, or home care is preferred.
Knowledge of CPR and first aid techniques is a plus.
Familiarity with creating and following care plans tailored to individual needs.
Strong communication skills and the ability to build rapport with clients.
Ability to perform heavy lifting as required by client needs.
Compassionate demeanor with a genuine desire to help others.
Must be able to pass background checks as required by state regulations.
