For over 75 years, we have served Interior Alaska by operating the region's only year-round emergency shelter while providing meals, case management, residential recovery support, and pathways toward stability. Position Summary
The Fairbanks Rescue Mission is seeking a compassionate, organized, and detail-oriented Supportive Services for Veteran Families (SSVF) Healthcare Navigator to assist Veterans and Veteran families participating in the SSVF Program. The Healthcare Navigator plays a vital role in connecting Veterans experiencing homelessness or housing instability with medical, behavioral health, and supportive care services through the U.S. Department of Veterans Affairs (VA) and community healthcare providers.
Working collaboratively with SSVF Case Managers, VA staff, healthcare providers, and community partners, the Healthcare Navigator helps ensure Veterans have timely access to VA healthcare, community medical services, behavioral health treatment, specialty care, and other supportive services. Through advocacy, care coordination, and service navigation, this position promotes improved health outcomes, increased self-sufficiency, and long-term housing stability for Veteran households.
Essential Duties and Responsibilities
Client Care & Navigation
- Assess participants' healthcare needs and develop individualized support plans.
- Assist Veterans with accessing VA healthcare, community medical services, behavioral health care, and health insurance programs.
- Coordinate medical, behavioral health, dental, vision, and specialty care appointments.
- Provide appointment reminders, transportation coordination, and follow-up support.
- Advocate for participants experiencing barriers to healthcare.
Service Coordination
- Collaborate with SSVF Case Managers and community partners to coordinate comprehensive services.
- Build relationships with VA staff, healthcare providers, and Veteran service organizations.
- Incorporate healthcare goals into participants' housing stability plans.
- Participate in case staffing, outreach, and community collaboration.
Documentation & Compliance
- Maintain accurate case notes and participant records in HMIS and other required systems.
- Prepare reports and ensure compliance with SSVF, VA, HUD, HIPAA, and agency requirements.
- Participate in discharge planning and coordinate continuity of care for program graduates.
- Perform other duties as assigned.
Qualifications
Required
- Associate's degree in Human Services, Social Work, Healthcare, Behavioral Health, or a related field, or equivalent experience preferred.
- Two (2) years of experience in case management, healthcare navigation, or social services.
- Strong communication, organizational, and documentation skills.
- Proficiency with Microsoft Office and electronic record systems.
- Valid Alaska Driver's License and ability to pass required background checks.
Preferred
- Bachelor's degree in a related field.
- Experience working with Veterans, individuals experiencing homelessness, or vulnerable populations.
- Knowledge of VA healthcare, SSVF, HUD, and HMIS.
Knowledge, Skills & Abilities
- Knowledge of trauma-informed care and community resources.
- Strong advocacy, problem-solving, and interpersonal skills.
- Ability to manage multiple priorities while maintaining confidentiality.
- Commitment to serving others with compassion, dignity, and respect consistent with Fairbanks Rescue Mission's Christian mission.
Work Environment
- Office-based position located at Fairbanks Rescue Mission with regular interaction with program participants, staff, community partners, and the U.S. Department of Veterans Affairs.
- Standard business hours (8:00 a.m.–4:00 p.m. or 9:00 a.m.–5:00 p.m.), with occasional evening or weekend meetings, trainings, or events as needed.
- Occasional local travel required to attend meetings, trainings, partner agencies, healthcare providers, and VA functions.
- Work involves interaction with individuals experiencing homelessness, behavioral health challenges, substance use disorders, and crisis situations requiring professionalism, sound judgment, and compassion.
