Full Job Description

This position involves working directly with individuals living with addiction, experiencing homelessness, and reentering society after incarceration to help them find attainable and realistic housing options.

As a supportive bridge housing provider, we are dedicated to assisting our participants in maintaining sobriety and achieving their employment and housing goals. We strive to create a nurturing and supportive environment where individuals can thrive and transition to permanent housing and stable employment.

Position Summary

We are seeking a dedicated and compassionate Housing Case Manager for our program. The ideal candidate will assist individuals in our transitional housing program in finding and securing safe, stable, and affordable housing.. This role involves working closely with clients in our homes to assess their needs, help them find and secure employment, identify available housing resources, and support them through the housing application and move-in process.

Key Responsibilities

Client Assessment and Support
Conduct comprehensive assessments of clients' housing needs and preferences.
Develop individualized housing plans in collaboration with clients.
Assist clients in resume writing, finding job openings, and applying to jobs.
Provide ongoing case management and support to help clients overcome barriers to housing stability.
Resource Identification and Coordination
Identify and maintain up-to-date knowledge of available housing resources, including emergency shelters, other transitional housing, permanent supportive housing, and affordable rental units.
Build and maintain relationships with landlords, property managers, and housing providers.
Coordinate and facilitate the housing application process, including assisting clients with completing applications, gathering necessary documentation, and attending housing interviews.
Documentation and Reporting
Maintain accurate and up-to-date client records and case notes.
Prepare and submit required reports and documentation in a timely manner.
Track and report on housing outcomes and program metrics.

Note: Additional responsibilities may be assigned as the needs of the clients evolve.

Qualifications

Experience
Minimum of 1 year of experience in housing navigation, or a related role.
Knowledge of local housing resources in Solano County, landlord-tenant laws, and the challenges faced by individuals experiencing homelessness.
Skills and Abilities
Excellent interpersonal and communication skills.
Strong organizational and time-management skills, with the ability to build trust and rapport with participants and landlords.
Ability to work independently and as part of a team.
Proficiency in Microsoft Office Suite and case management systems.
Valid driver’s license and reliable transportation.

Working Conditions

Full-time position, approximately 40 hours per week.
9AM-5PM. or 10AM-6PM.
Travel within the community to meet with clients and housing providers.
Much of the work will be done at our housing locations to meet with program participants.
