Position Summary
Heart of Hope Home Health Care is seeking a highly skilled, compassionate, and motivated Licensed Mental Health Professional (LMHP) to provide clinical leadership and direct therapeutic services within our Mental Health Partial Hospitalization Program (MH-PHP) serving adults ages 18-65 experiencing serious mental illness, emotional instability, psychiatric symptoms, and behavioral health challenges.
The LMHP is responsible for conducting comprehensive clinical assessments, developing individualized treatment plans, facilitating individual and group psychotherapy, completing clinical documentation, coordinating care, managing crisis interventions, and ensuring compliance with all applicable Virginia Department of Behavioral Health and Developmental Services (DBHDS) and Department of Medical Assistance Services (DMAS) regulations.
The LMHP plays a critical role in helping individuals stabilize psychiatric symptoms, improve daily functioning, prevent hospitalization, and successfully transition to lower levels of care through evidence-based, recovery-oriented treatment services.

Essential Duties and ResponsibilitiesClinical Assessment & Treatment Planning
- Conduct comprehensive biopsychosocial assessments, mental health evaluations, and risk assessments.
- Assess individuals for admission, continued stay, transfer, and discharge from the MH-PHP program.
- Develop individualized service plans (ISPs), treatment plans, and discharge plans in collaboration with the individual and treatment team.
- Identify clinical needs, strengths, barriers, risk factors, and recovery goals.
- Complete suicide risk assessments, safety plans, and crisis prevention plans as needed.
- Conduct ongoing reassessments to determine treatment progress and medical necessity.
Individual & Group Therapy
- Provide evidence-based individual psychotherapy services.

Crisis Intervention & Risk Management
- Assess and manage psychiatric crises, suicidal ideation, homicidal ideation, self-harm behaviors, and acute behavioral health emergencies.
- Provide immediate intervention and stabilization services.
- Coordinate emergency evaluations, hospitalization referrals, and crisis services when clinically indicated.
- Ensure all incidents are documented and reported according to DBHDS regulations and agency policy.
- Participate in multidisciplinary staffing and case consultations.
Care Coordination
- Coordinate treatment with psychiatrists, nurse practitioners, primary care physicians, hospitals, community service boards (CSBs), case managers, residential providers, and other healthcare professionals.
- Collaborate with family members, legal guardians, probation officers, social service agencies, and community supports when authorized.
- Facilitate referrals to community resources and step-down services.
- Assist individuals in accessing housing, employment, transportation, benefits, and supportive services.
Documentation & Compliance
- Complete all required clinical documentation within established agency timelines.
- Maintain accurate, timely, and compliant records in the Electronic Health Record (EHR).

- Assist in identifying barriers to treatment and developing corrective interventions.
- Collaborate with psychiatric providers regarding medication adherence, symptom management, and treatment progress.
Program Development & Leadership
- Assist with program implementation and continuous quality improvement initiatives.
- Provide clinical consultation and support to QMHPs, residents, interns, and other behavioral health staff as assigned.
- Participate in staff training and professional development activities.
- Support agency accreditation, licensing, and compliance efforts.
Minimum Qualifications
Applicants must possess one of the following Virginia credentials:
- Licensed Professional Counselor (LPC)
- Licensed Clinical Social Worker (LCSW)
- Licensed Marriage and Family Therapist (LMFT)
- Licensed Clinical Psychologist
- Licensed Substance Abuse Treatment Practitioner (LSATP) with qualifying mental health experience
- Licensed Mental Health Professional (LMHP) as defined by Virginia regulations
Must maintain an active and unrestricted Virginia professional license.

- Minimum two (2) years of experience providing behavioral health services to adults.

The LMHP shall ensure services support the goals of Partial Hospitalization treatment, including psychiatric stabilization, symptom reduction, skill development, and prevention of inpatient psychiatric hospitalization. Virginia regulations require partial hospitalization programs to provide intensive structured treatment, including individual and group therapy, treatment planning, and coordination of psychiatric services.
