Job Summary

Welcome to Central Healthcare of Le Center

Looking for a Part time PT to join our team

Join our dynamic healthcare team as a PRN (as-needed) Physical Therapist, where your expertise will directly impact patient recovery and well-being. In this role, you will provide specialized physical therapy services across various settings, including outpatient clinics, inpatient units, and home health environments. Your dedication to patient-centered care and clinical excellence will help improve mobility, reduce pain, and promote independence for diverse patient populations. This flexible position offers the opportunity to work with a variety of cases while maintaining a balanced schedule that fits your lifestyle.

Duties

Assess patients’ physical conditions through comprehensive evaluations, including physiology and anatomy knowledge, to develop personalized care plans.
Implement evidence-based therapy techniques such as trigger point therapy, orthopedics, neurology interventions, and geriatrics-focused treatments.
Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless patient transitions across care settings.
Document all patient interactions accurately in electronic health records (EHR) systems in accordance with clinical documentation standards.
Provide therapeutic interventions tailored for outpatient rehabilitation centers, inpatient hospital units, or home & community care environments.
Educate patients and caregivers on physiotherapy exercises, mobility strategies, and injury prevention techniques.
Monitor progress regularly and adjust treatment plans to optimize outcomes while adhering to medical terminology and clinical protocols.

Requirements

Valid state licensure as a licensed Physical Therapist with practicing in a physical therapy role.
Demonstrated experience in acute care, outpatient clinics, inpatient hospital settings, or home health environments.
Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards.
Proven ability to work effectively with geriatric patients, pediatric populations, and individuals with neurological or orthopedic conditions.
Experience managing care plans for diverse patient needs including outpatient rehabilitation, inpatient hospital stays, or home & community care services.
Familiarity with electronic health records (EHR) or electronic medical records (EMR) systems for documentation and patient management.
Excellent communication skills to collaborate with healthcare teams and educate patients on physiotherapy techniques.
Ability to handle discharge planning processes efficiently while maintaining high standards of patient care.
