Speech Language Pathologist (SLP) – Home Health (PRN Opportunity)

We are seeking compassionate, dedicated professionals who bring both strong clinical foundations and a genuine heart for patient care.

Qualifications

We are looking for an SLP who meets one of the following:

Holds a Certificate of Clinical Competence (CCC) in Speech-Language Pathology or Audiology from ASHA, or
Meets the educational requirements for ASHA certification and is actively completing supervised experience.
Current state license to practice in Florida.
Minimum two (2) years of clinical experience preferred.
Strong communication, organizational, and clinical documentation skills.
Experience in home health is a plus, but not required—we provide support every step of the way.

Your Impact & Responsibilities

As a Speech Language Pathologist with A Care Connection, you will play an essential role in helping patients regain functional communication and enhance their quality of life at home. Your responsibilities include:

Clinical Care

Provide a full range of Speech-Language Pathology services as ordered by the physician.
Conduct initial evaluations, including history, assessment, and development of a personalized treatment plan.
Improve or maximize patient communication abilities and functional independence.
Perform skilled therapy procedures and adjust treatment approaches as needed.
Assist the physician in evaluating the patient’s level of function and progress.

Care Planning & Documentation

Develop and update individualized plans of care; revise goals as patient needs evolve.
Prepare accurate clinical and progress notes for each visit.
Complete and submit all required OASIS assessments, reassessments, resumptions of care, transfers, discharges, and significant-change documentation within agency timelines.
Utilize appropriate ICD-9/ICD-10 coding.

Collaboration & Supervision

Participate in interdisciplinary care planning with nurses, therapists, aides, and physicians.
Provide direction and supervision to assigned personnel.
Conduct supervisory visits every 14 days for patients receiving home health aide services.
Consult with physicians regarding changes in treatment or patient condition.
Educate patients and family members on home programs and strategies to continue progress.
Provide occasional in-service training for A Care Connection staff.

Continuity of Care

Submit evaluations and treatment plans for physician approval, with recertifications completed every 60 days.
Document progress, reassessments, and discharge summaries as treatment milestones are reached.
