Full Job Description

Job Summary

The Physical Therapist (PT) provides one-on-one, skilled, patient-centered therapy

services in the home setting, helping individuals improve mobility, relieve pain, increase

strength, balance, and overall functional independence within their own environment.

This role focuses on evaluating patients in their homes, identifying functional limitations,

and developing individualized treatment plans that support safe mobility, fall prevention,

and the ability to perform activities of daily living.

The PT works closely with patients, caregivers, physicians, and the interdisciplinary

home health team to establish realistic goals, monitor progress, and adjust interventions

as patient needs evolve. Education is a key component of this role, including instruction

on home exercise programs, safety strategies, and adaptive techniques to promote

long-term independence. Through skilled, compassionate care, the Physical Therapist

plays an essential role in improving quality of life and reducing hospitalizations.

Requirements and Qualifications:

Education: Graduate of an accredited Physical Therapy program
Current, active, unencumbered Arizona Physical Therapist license or equivalent (compact privilege)
Current BLS/CPR Certification.

Minimum Qualifications:

Knowledge of anatomy, physiology, and therapeutic exercise principles.
Strong communication and interpersonal skills to effectively educate patients and collaborate with caregivers and healthcare teams.
Ability to safely perform physical tasks required for in-home patient care, including mobility assistance and transfers.
A valid driver’s license, reliable transportation, valid auto insurance, and ability to travel to patient’s homes, assisted living facilities or group homes. homes as needed.
Develops realistic and appropriate short and long-term goals; discusses evaluation results and goals with patients

Preferred Qualifications:

Previous experience in home health or community-based physical therapy.
Familiarity with OASIS, Medicare guidelines, and home health regulations.
Experience working with geriatric or medically complex patients.
Proficiency with electronic medical record (EMR) systems.
Training in fall prevention, balance training, or and functional mobility interventions.
Strong interpersonal skills for educating patients and collaborating with care teams.
Bilingual abilities to support diverse patient populations.

Key Responsibilities:

Perform Conduct comprehensive in-home evaluations to assess mobility, strength, balance, functional status, and safety risks.
Develop and implement individualized treatment plans aligned with physician orders.
Develops realistic and appropriate short and long-term goals; discusses evaluation results and goals with patients, family, primary medical provider and other concerned persons as appropriate.
Provide skilled, evidence based therapeutic interventions, including therapeutic exercise, gait training, balance training, and functional mobility activities.
Monitor patient progress and modify plans of care as clinically indicated.
Educate patients and caregivers on home exercise programs, safety strategies, fall prevention, and energy conservation techniques.
Collaborate with physicians, nurses, case managers, and other members of the home health interdisciplinary team to coordinate care.
Complete accurate and timely documentation in accordance with agency policies and regulatory requirements.
OASIS Documentation: Complete specialized Medicare-required documentation
(Outcome and Assessment Information Set) to track patient progress and ensure regulatory compliance.
Provides clinical work direction for the physical therapist assistant and delegates to them appropriate duties that are within their abilities. Monitors PTA performance and supervises within regulatory guidelines provided by state board and/or CMS.
Participate in care coordination, case conferences, and ongoing quality improvement efforts.
Maintain compliance with professional standards, infection control practices, and continuing education requirements.
Presents professional appearances and demeanor.

Essential Skills:

The Physical Therapist must demonstrate:

Ability to manage a caseload independently in a home setting and strong clinical assessment skills to identify functional deficits and safety concerns unique to the home environment.
Knowledge and application of professional practice and regulatory requirements.
Effective communication for educating patients and caregivers and collaborating with interdisciplinary team members.
Physical stamina, sound clinical judgment, and adaptability to provide safe, effective care in varied home settings.
Proficiency with documentation systems and an understanding of home health regulations to support the delivery of high-quality, patient-centered therapy services.
Provides accurate and timely documentation for third party reimbursement purposes.
Ability to work in demanding work environment, organize multiple priorities, complete work duties within expected timelines and requirements.
Consistently follows infection control and universal precautions procedures as applicable to the work at hand.
