Job description:

This position provides on-call care management to assist our clients in managing and navigating their health and wellness care.

The qualified candidate will be professional, have knowledge of health care systems and skills in navigating them, familiarity with the local resources for seniors, possess the ability to relate easily and compassionately with older adults, and have the capability to be discreet and ethical in safeguarding clients' confidentiality.

The required responsibilities, Knowledge, Skills and Abilities:

Knowledge of the local health care system (physicians, hospitals, assisted living communities, skilled care facilities, Medicare/Medicaid, clinics, resource centers, etc.), adept at facilitating health care coordination.
Skilled in quickly assessing member needs and identifying resources and solutions.
Strong interpersonal and communication skills, writing and computer skills, assertiveness, professionalism, and a positive, upbeat, and patient disposition.
Demonstrated problem solving skills using business acumen including the ability to use sound judgment.
Ability to work independently, determine priorities, work collaboratively, and make appropriate decisions.
Skilled at establishing credible and trusted relationships with members, family, other designees, and health care providers.
Able to relate easily and compassionately with seniors and families, provide non-directive leadership to guide member’s decision-making, facilitate constructive relationships, and mediate and resolve conflict.
Able to navigate clearly and discern accurately the boundaries of one’s role: as an enabler of the actions and decisions of others, not the decision-maker, as a facilitator of health care, not a provider.
Discrete, ethical, careful in safeguarding confidentiality per HIPAA, attentive to detail, time management skills, organizational skills, prompt and timely, and committed to customer service.
Ability to handle stress and stressful situations

Licensure and Experience Required:

Registered Nurse with experience in case management and/or discharge planning.
Minimum of bachelor’s degree in Nursing with applicable master’s degree preferred.
Experience with geriatric patients desired.

ESSENTIAL JOB RESPONSIBILITIES & DUTIES:

Essential Geriatric Care Supervisor Functions
Provide on-call support and after hours care coordination
Exercises critical thinking skills for the provision of health care management, advisory, and advocacy services to clients.
Strives to facilitate a relationship with clients, families, providers, and others, promoting improved communications and fostering clients’ greater well-being, independence, and peace of mind.
Responsibilities include:
Advocate for the client, provide medical information, communication with client contacts regarding the client’s medical status and documentation of all activities and communication.
Monitoring client’s health status and well-being, recommending resources and solutions as appropriate.
Transportation of clients for health care services and accompaniment to urgent/emergency room visits.
Liaison and coordinator with family members, designees, and providers; creation and maintenance of clients’ personal health care record using Internet based software; linkage to other services offered by Take Care Advisor.
Performance of administrative tasks associated with service to members including handling office phones, scheduling as it relates to client appointments, documentation of all activity/actions and contact with clients and/or designees, providing education and information, and regular communication to clients and/or designees, and fulfillment of all designated tasks in a through, timely, and reliable manner.
Collaborating and communicating with the on-call Supervisor as needed for direction and guidance.
Written and verbal reports on clients after each on-call segment.
Participation in Take Care Advisor team meetings as appropriate.
Participation in an on-call rotation with a minimum of 2 weekdays and 2 weekend days per month (minimum of 4 call segments per month).
