JPATIENT NAVIGATOR – NYS DEPARTMENT OF HEALTH SOCIAL CARE NETWORKPosition Summary

The Patient Navigator supports individuals in accessing health‑related social care services through the New York State Department of Health (NYSDOH) Social Care Network. This role helps patients identify needs, understand available resources, and successfully connect to community‑based organizations (CBOs), care management agencies, and health system partners. The Navigator ensures that individuals receive timely, coordinated, and person‑centered support to improve health outcomes and reduce barriers to care.

Key ResponsibilitiesPatient Engagement & Assessment

Conduct outreach to patients referred through the Social Care Network, managed care plans, hospitals, clinics, and community partners.
Complete structured social needs assessments to identify challenges related to housing, food access, transportation, benefits, behavioral health, caregiving, and other social determinants of health.
Build rapport with patients using trauma‑informed, culturally responsive communication.

Navigation & Coordination

Assist patients in understanding available social care services and how to access them.
Provide warm handoffs to CBOs, care managers, and service providers.
Coordinate appointments, follow‑ups, and referrals to ensure continuity of care.
Track referral outcomes and support patients until needs are resolved or stabilized.

Education & Empowerment

Educate patients on available benefits, community programs, and self‑advocacy strategies.
Support patients in completing applications for services such as SNAP, HEAP, Medicaid, housing programs, transportation assistance, and other supports.
Encourage patient engagement in preventive care, chronic disease management, and wellness activities.

Documentation & Compliance

Maintain accurate, timely documentation in NYSDOH Social Care Network platforms and agency systems.
Record assessments, referrals, follow‑up actions, and outcomes according to NYSDOH standards.
Protect patient privacy and comply with HIPAA, NYSDOH policies, and agency protocols.
Participate in quality assurance activities, audits, and performance improvement initiatives.

Collaboration & Communication

Work closely with hospitals, primary care practices, behavioral health providers, CBOs, and care management agencies.
Participate in interdisciplinary meetings and case reviews.
Communicate effectively with patients, families, caregivers, and providers to ensure coordinated support.

Qualifications

Bachelor’s degree in Social Work, Human Services, Public Health, Psychology, or related field preferred; relevant experience may substitute.
Experience working with diverse populations, including individuals with complex social needs, chronic conditions, disabilities, or behavioral health challenges.
Knowledge of New York State social service systems, Medicaid programs, and community resources.
Strong communication, motivational interviewing, and problem‑solving skills.
Ability to manage multiple cases, prioritize tasks, and work independently.
Proficiency with electronic documentation systems and comfort with technology.
Bilingual skills a plus.
