Our services are designed to address the whole person by helping individuals access behavioral health treatment, medical care, housing, transportation, employment, education, benefits, recovery resources, legal and reentry supports, and other necessary community services.
Sanctuary of Purpose works with individuals who may have complex needs and multiple barriers to stability. Our Case Managers are expected to provide compassionate, professional, ethical, person-centered, trauma-informed, and highly organized care coordination while maintaining clear professional boundaries and complete documentation.
POSITION SUMMARY
Sanctuary of Purpose is seeking a highly motivated, organized, compassionate, and dependable Behavioral Health Case Manager to provide comprehensive case management, resource coordination, advocacy, referral, monitoring, follow-up, and service-planning support to clients.
The Case Manager will assist clients in identifying needs, establishing individualized goals, locating appropriate community resources, accessing services, addressing barriers to care, coordinating with providers and community agencies, and monitoring progress toward increased stability and independence.
This position requires extensive client interaction, community resource knowledge, professional communication, accurate documentation, and the ability to work effectively with individuals who may be experiencing significant behavioral health, recovery, housing, financial, employment, transportation, legal, or social challenges.
Case Managers must remain within their education, training, credentials, assigned scope of practice, Sanctuary of Purpose policies, Health First Colorado requirements when applicable, and the direction and oversight of the appropriate qualified or rendering provider.
REQUIRED EDUCATION AND CREDENTIALS
Bachelor’s Degree Required
A bachelor’s degree is a mandatory minimum qualification for this Behavioral Health Case Manager position. Applicants who do not possess a qualifying bachelor’s degree will not be considered for this specific role.
The applicant must possess a Bachelor’s degree in Case Management or a closely related behavioral health, social services, or human services field from an accredited college or university. Qualifying degrees may include Case Management, Social Work, Psychology, Counseling, Human Services, Behavioral Health, Behavioral Science, Sociology, Human Development, Family Studies, Rehabilitation Services, Addiction Studies, Community Health, Public Health, Nursing, or another closely related field.
Applicants with a bachelor’s degree in an unrelated field may only be considered when they have at least one year of documented behavioral-health, case-management, social-services, or closely related experience and otherwise meet applicable payer and organizational requirements.
Higher-level education or licensure may be accepted when applicable. Examples include an MSW, LSW, LCSW, LPC, LPC Candidate, LAC, LMFT, or another appropriately qualified behavioral-health credential.
Degree Verification Required
The selected applicant must provide acceptable documentation verifying completion of the required bachelor’s degree. Sanctuary of Purpose may request transcripts, a diploma, institutional degree verification, applicable professional licenses or certifications, and documentation of relevant experience. Any employment offer may be contingent upon successful verification.
Possession of a clinical license does not authorize an employee to perform services outside the employee’s lawful scope of practice, assigned responsibilities, payer requirements, or Sanctuary of Purpose policies.
IMPORTANT CREDENTIALING NOTICE
Colorado does not issue a single universal professional license titled “Case Manager.”
Eligibility to perform Medicaid-reimbursable case-management services is determined by the specific service being delivered, the employee's education or professional credentials, the organization's provider enrollment, applicable Health First Colorado requirements, payer requirements, and required rendering-provider oversight.
Sanctuary of Purpose will verify education, professional credentials, experience, exclusions, and other qualifications before an employee is authorized to provide Medicaid-related services.
Being hired by Sanctuary of Purpose does not independently authorize an individual to bill Medicaid or practice as a licensed behavioral-health professional.
PRIMARY JOB RESPONSIBILITIES
The Case Manager will provide individualized case-management services based on each client’s assessed needs, goals, authorized service plan, and applicable payer requirements.
Assess and document barriers affecting behavioral health, substance use recovery, medical care, medication access, housing, homelessness, food security, transportation, employment, education, Medicaid, Social Security, disability resources, identification documents, legal or reentry obligations, probation or parole requirements, family reunification, financial stability, daily living, recovery support, and community integration.
Work collaboratively with clients to identify realistic steps for addressing barriers and improving stability.
- Individualized Service Planning
Develop and maintain client-centered case-management plans that identify goals, barriers, action steps, resources, referrals, reasonable timelines, and follow-up needs. Monitor progress, identify changes in circumstances, update interventions when needed, and support the client’s participation in decision-making. Plans must be individualized and based on the client’s actual needs.
- Referral and Resource Coordination
Connect clients to appropriate community services, which may include primary care, behavioral health treatment, therapy, psychiatry, substance use treatment, IOP, recovery programs, detoxification services, medication-assisted treatment, hospitals, community mental health programs, housing and shelter programs, rental or utility assistance, food and SNAP resources, Medicaid and Social Security resources, disability services, workforce centers, vocational rehabilitation, GED and education programs, transportation and NEMT, legal aid, reentry programs, probation or parole resources, identification-document assistance, and other community supports.
Effective case management requires more than giving a client a phone number. When appropriate, assist clients in understanding referrals, completing necessary steps, resolving barriers, communicating with agencies, arranging appointments, and following up on referral outcomes.
Coordinate services with authorized members of the client’s care team, including therapists, counselors, medical providers, peer professionals, outreach staff, QBHAs, housing staff, social workers, probation or parole officers, hospitals, treatment programs, benefits specialists, employment specialists, community agencies, and authorized natural supports. All communication must comply with confidentiality and release-of-information requirements.
Follow up on referrals and service plans to determine whether appointments were scheduled, services were accessed, barriers remain, goals are progressing, or alternative resources are needed. Identify significant changes in client circumstances and communicate relevant information to authorized members of the service team.
- Housing Stability Support
Assist with housing applications, affordable-housing searches, documentation gathering, rental-assistance resources, homelessness resources, identification documents, and barriers involving criminal or rental history. Case Managers may assist with housing navigation but may not guarantee placement, approval, benefits, or eligibility.
- Employment and Vocational Support
Assist clients with workforce-center referrals, employment applications, resume resources, interview preparation, vocational rehabilitation, job-training programs, apprenticeships, education, GED programs, transportation-to-employment resources, and other employment-related community services.
- Benefits and Basic-Needs Coordination
Assist clients in accessing appropriate resources involving Medicaid, SNAP, Social Security, SSI/SSDI referrals, food banks, clothing, transportation, medical and dental services, identification documents, birth certificates, and other basic-needs resources. Case Managers may not represent themselves as attorneys, financial advisors, disability examiners, or government eligibility workers unless separately qualified and authorized.
MEDICAID DOCUMENTATION RESPONSIBILITIES
When services are being provided under Health First Colorado or another payer, the employee must meet all documentation requirements applicable to the service.
Documentation must accurately demonstrate:
- Who received the service.
- Date of service.
- Location or approved place of service.
- Start and stop time when required.
- Actual duration of service.
- Reason for the contact.
- Client's identified need.
- Case-management intervention performed.
- Resources or providers contacted.
- Actions completed by the Case Manager.
- Client participation and response.
- Progress toward the client's goals.
- Barriers identified.
- Follow-up required.
- Plan for continued case-management services.
- Coordination with other providers when applicable.
Documentation must demonstrate the actual work performed.
Employees may not submit generic, copied, falsified, inflated, duplicated, or otherwise inaccurate documentation.
DOCUMENTATION DEADLINE
All billable documentation must be completed within the timeframe established by Sanctuary of Purpose policy and applicable payer requirements.
Sanctuary of Purpose may require documentation to be entered within 24 hours of the service being provided.
Failure to timely complete documentation may result in the service being non-billable and may result in corrective action.
TIME AND BILLING INTEGRITY
Case Managers are responsible for accurately reporting the time actually spent performing authorized services.
Employees may never:
- Bill for services that did not occur.
- Record more time than was actually provided.
- Bill two clients for the same individual service time.
- Duplicate another provider's service.
- Alter documentation to increase reimbursement.
- Document client contact that did not occur.
- Submit copied notes that do not describe the individual service.
- Bill transportation time when transportation itself is not an allowable case-management service.
- Bill time spent simply waiting for a client during an appointment when the time is not otherwise a covered case-management activity.
Any suspected billing irregularity must immediately be reported to management or the organization's compliance representative.
SCOPE OF PRACTICE
Case Managers provide case management, not psychotherapy or independent clinical treatment unless the individual holds a separate professional license and has specifically been authorized by Sanctuary of Purpose to provide those services.
Unless separately credentialed and authorized, a Case Manager shall not:
- Diagnose mental health conditions.
- Diagnose substance use disorders.
- Provide psychotherapy.
- Independently develop clinical diagnoses.
- Represent themselves as a licensed therapist.
- Provide medical advice.
- Prescribe medication.
- Alter medication instructions.
- Perform services requiring a professional license they do not possess.
- Provide legal advice.
- Guarantee benefits.
- Guarantee housing.
- Guarantee Medicaid eligibility.
- Practice outside their training or assigned responsibilities.
Employees must immediately refer clinical, medical, legal, safety, or other issues beyond their competence to the appropriate qualified professional.
CONFIDENTIALITY AND PRIVACY
The Case Manager must maintain strict confidentiality regarding client information.
The employee must comply with all applicable organizational and legal requirements involving:
- HIPAA
- Protected Health Information
- Confidentiality
- Electronic communications
- Secure documentation
- Release-of-information requirements
- Minimum-necessary standards
- 42 CFR Part 2 when applicable to substance-use-disorder records
- Sanctuary of Purpose privacy policies
Client information may only be accessed, discussed, transmitted, or disclosed when permitted by law, authorization, organizational policy, and job responsibilities.
PROFESSIONAL BOUNDARIES
Case Managers must maintain appropriate professional boundaries at all times.
Employees may not:
- Enter inappropriate personal relationships with clients.
- Enter sexual or romantic relationships with clients.
- Borrow money from clients.
- Loan money to clients without explicit program authorization.
- Accept inappropriate gifts.
- Sell personal items or services to clients.
- Buy controlled or prohibited items for clients.
- Share unnecessary personal information.
- Engage clients in personal business ventures.
- Exploit clients financially, emotionally, or otherwise.
- Communicate with clients through unauthorized channels.
- Use their position for personal benefit.
CRISIS AND SAFETY RESPONSIBILITIES
The Case Manager must be capable of identifying situations requiring immediate escalation.
This may include:
- Threats of harm
- Medical emergencies
- Psychiatric emergencies
- Suspected overdose
- Significant intoxication
- Domestic violence concerns
- Abuse or neglect
- Dangerous behavior
- Housing emergencies
- Significant behavioral deterioration
- Other immediate safety concerns
Case Managers must follow Sanctuary of Purpose emergency procedures and promptly notify appropriate supervisors, emergency services, clinical personnel, or other authorities when required.
Case Managers are not expected to perform services outside their training or professional scope during a crisis.
MANDATORY REPORTING
Employees must comply with Colorado mandatory-reporting obligations that apply to them based on their professional role, credentials, client population, and circumstances.
Employees must immediately consult a supervisor when they believe a report may be legally required.

EXPERIENCE PREFERRED
Preference is given to candidates with 1–3 years of experience in case management, behavioral health, Medicaid services, mental health, substance use recovery, homelessness, supportive or transitional housing, justice-involved populations, probation/parole, reentry, social services, care coordination, or community outreach.
ADDITIONAL EMPLOYMENT REQUIREMENTS
Candidates must complete education and credential verification, applicable background and exclusion screening, HIPAA and confidentiality training, Medicaid documentation/compliance training, Sanctuary of Purpose orientation, required supervision and staff meetings, and continuing training. Employees must maintain required credentials, dependable attendance, and reliable communication.
TRANSPORTATION REQUIREMENTS
Reliable transportation may be required for community-based services. When driving is an essential function, candidates may be required to maintain a valid driver’s license, acceptable driving record, current automobile insurance, and reliable vehicle. Client transportation requires separate Sanctuary of Purpose authorization.

SUPERVISION
Case Managers must participate in required administrative, programmatic, compliance, and/or clinical oversight. Supervision may include case review, documentation review, compliance review, crisis or boundary consultation, resource consultation, performance coaching, and quality assurance. Employees must seek guidance when a situation exceeds their competence, training, or assigned role.
