Full Job Description

Essential Responsibilities:

DSP- PAC Service Provider (In Home)

· Assists in the implementation of individual plans for individuals and behavioral plan goals;

· Assists individuals in the pursuit of recreational, social and community activities based on their preferences and individual plans;

· Assists and encourages individuals in the establishment of social networks;

· Promotes participation and involvement in community outings;

· Works all assigned schedules, including but not limited to: arriving and departing on time, requesting time off in advance, and notifying your supervisor of changes to your assigned work schedule;

· Ensures each individuals’ rights are protected;

· Treats individuals with dignity and respect;

Activities of Daily Living

· Administers medications to individuals as assigned;

· Observes individuals for any changes in behavior or health that may have resulted from medication administration;

· Ensures individuals are dressed neatly and appropriately and are groomed at all times;

· Assists and encourages individuals to care for immediate personal needs such as toileting, washing their hands, and eating;

· Keeps location – both interior and exterior – clean, organized, and tidy and encourage participants to participate in upkeep as appropriate according to their individual plans;

· Teaches individuals daily skills as appropriate according to participant’s individual plan;

Communication

· Observes participants for evidence of injury or bruises and evaluates for changes in emotional and physical status;

· Reports any noted problems to the Day Services Manager immediately and completes a written incident report form;

· Reports all medical related incidents to the Day Services Manager;

· Acts as a representative for DDSi/New Adventures 4 You in a professional manner when required, to families, service coordinators, State officials, community partners, local businesses, and employers;

· Represents DDSi/New Adventures 4 You in a professional manner to families, service coordinators, State officials, and community partners when required;

· Maintains participant confidentiality;

Teamwork

· Assists the Director of Operations and Director of Support Services in the preparation for the annual (PCISP) Individual Planning meetings and quarterly meetings;

· Treats all individuals and coworkers and with dignity, respect, and fairness;

· Provides support to other team members when individuals are having behavioral issues;

Documentation

· Documents all program data, including but not limited to: individual plan data, behavior plan data, community integration logs, assessments, mileage logs, maintenance requests forms, and supply acquisition forms;

· Documents individuals health (i.e. seizure charts, bowel movements, fluid intake.);

· Ensures that all documentation is completed accurately and in a timely manner;

· Ensures Medication Administration Record (MAR) entries are completed daily; when applicable

· Clocks in and out before and after every shift and submits Leave Request forms when applicable;

Physical Requirements

· Physically able to perform the essential functions of the job as described above including: walking, bending, stooping, kneeling, squatting, sitting, reaching overhead and lifting up to fifty (50) pounds.

· Meet applicable federal, state and/or local health requirements relating to communicable disease.

· Must satisfactorily complete the agency program for physical management of aggressive behavior and maintain skills in this area as assessed in annual refresher classes.

· Must satisfactorily complete the agency health and safety program and maintain skills in this area as assessed in annual training.

Qualifications

· Ability to communicate effectively; orally and in writing.

· High School diploma or GED

· Ability to use common word-processing and communications software programs within a Windows environment.

· Satisfactory completion of a criminal background check as required by federal and/or state laws and any other post-offer pre-employment screenings as mandated by a governing agency or DDSi/New Adventures 4 You.

· Valid Indiana driver’s license, a good driving record and a current automobile liability insurance policy are required.

Additional Responsibilities and Standards for Performance

· Must maintain a punctual and good attendance record.

· Must possess or have the ability to acquire through staff training a good working knowledge of procedures pertaining to: consumer rights, abuse and neglect, and Medicaid reimbursement.

· Must maintain a valid Indiana motor vehicle operator’s license, an acceptable driving record and automobile insurance acceptable to the State of Indiana and provide documentation.

· Must maintain current training and knowledge in CPR/First Aid and provide documentation.

· Must update tuberculosis test (TB) annually and provide documentation.

· Ability to relate well with consumers, families and other program staff.

· Ability to remain calm in stressful situations.

· Must be able to exercise judgment and initiative in providing services to consumers.

· Must be able to follow oral and written direction.
