Full Job Description

Job Summary
We are seeking an energetic and dedicated Nurse Manager to lead and coordinate nursing services within a dynamic healthcare environment. In this pivotal role, you will oversee clinical operations, ensure high-quality patient care, and foster a collaborative team atmosphere. Your leadership will drive compliance with healthcare standards, optimize patient outcomes, and support staff development. This position offers an exciting opportunity to make a meaningful impact on patient lives while managing a skilled nursing team in a fast-paced setting.

Duties

Work alongside the Director of Nursing, leading and supervising nursing staff, including hiring, training, performance evaluations, and ongoing professional development.
Oversee daily clinical operations ensuring adherence to facility policies, safety protocols, and regulatory CMS standards
Collaborate with IDT as well as physicians to develop a plan of care
Manage patient assessments, vital signs monitoring, medication administration, IV infusion procedures, catheter care, and other clinical interventions with precision and compassion.
Ensure accurate medical documentation using EMR (Electronic Medical Records) systems like Point Click Care, and maintain comprehensive medical records for legal compliance.
Facilitate discharge planning, case management, utilization review, and utilization management to optimize patient flow and resource utilization.
Promote infection control practices and aseptic techniques to maintain a safe environment for patients and staff.
Support quality improvement initiatives and participate in QAPI meetings weekly

Requirements

Valid RN/LPN license with current registration; RN preferred.
Extensive experience in subacute setting, skilled nursing, short term rehabilitation
Proven leadership skills with supervising experience in nursing management or team management roles.
Strong knowledge of EMR/EHR systems, including Point Click Care and EPIC
Critical care experience
Familiarity with complex medical needs
Excellent understanding of physiology knowledge related to patient assessment and anatomy knowledge for accurate diagnostics.
Ability to work effectively with individuals with varying cognitive and functional limitations
Knowledge of HIPAA regulations to ensure confidentiality of patient information; basic life support (BLS) certification required.
Strong communication skills for triage processes, family planning discussions, discharge planning coordination, and health coaching.
