Part-Time Care Advocate / Aging Life Care Professional
15-25 hours per week, W2 Employee
abridge: Aging Life Care Management is a client-focused, results-driven, holistic care management agency based in Athens, Georgia, serving clients throughout Greensboro and surrounding communities.
We are seeking a highly skilled, compassionate, and independent Care Advocate to join our team. This position is ideal for an experienced professional who understands the complexities of aging, can navigate complicated family dynamics, and is confident leading a care management process on behalf of clients and their families.
This is not a task-focused position. We are looking for someone who can assess complex situations, identify what needs to be done, develop and implement thoughtful solutions, coordinate professionals, communicate effectively, and advocate for the client's goals and quality of life.
The Role
The Care Advocate provides individualized care management to older adults and their families, serving as an objective professional resource for healthcare, aging, and ongoing care-planning needs.
You will:
- Conduct comprehensive client assessments and identify needs using the ALCA 8 Domains of Expertise.
- Develop individualized care plans based on the client's goals, needs, preferences, and circumstances.
- Establish measurable goals and coordinate appropriate actions and services.
- Monitor client progress and adjust recommendations as circumstances change.
- Serve as a professional liaison among families, healthcare providers, caregivers, and community resources.
- Coordinate and advocate for services to ensure the client's needs are being appropriately addressed.
- Make appropriate referrals to professional services and connect clients and decision-makers with community resources.
- Provide family caregiver coaching and guidance when appropriate.
- Conduct in-person client visits, with a minimum of one visit per month depending on client needs and preferences.
- Maintain thorough, accurate, and timely documentation of assessments, recommendations, communications, and care-management activities.
- Respond to client and family needs with appropriate urgency while maintaining sound professional judgment.
- Maintain strict confidentiality, HIPAA compliance, and professional boundaries.

Required Education & Experience
- Master's degree preferred in nursing, psychology, gerontology, applied sciences, social work, or a related field.
- Minimum 2–3 years of direct-care and/or care-management experience, including experience working with older adults.
- Demonstrated knowledge of aging, dementia, movement disorders, and chronic disease management.
- Experience navigating complex family systems and family dynamics.
- Strong written and verbal communication skills.
- Excellent documentation and organizational skills.
- Ability to exercise sound judgment and make decisions independently.
- Healthy work-life balance skills
Preferred Certifications
Preference will be given to candidates with professional certifications such as:
- Certified Care Manager (CCM)
- Care Manager Certified (CMC)
- Certified Advanced Social Work Case Manager (C-ASWCM)
- Certification in Gerontology
- Other relevant care-management or gerontology certifications
Employees are responsible for maintaining required licenses, certifications, and continuing education. The company may, at its discretion, assist with certain professional-development expenses.
Licensure & Transportation
- Valid Georgia driver's license required.
- Reliable personal transportation is required for client visits, professional meetings, and other company-related appointments.
Technology & Documentation
Because this position is performed with a high degree of independence, the successful candidate must be comfortable using technology and learning new systems.
Proficiency with the following is expected:
- Microsoft Office 365, particularly Outlook email and calendar
- Microsoft Teams or comparable meeting platforms
- OneDrive or comparable cloud-based file systems
- Web-based databases and care-management systems
- Internet research and resource identification
- Business communication applications
The Care Advocate will be trained on company-specific systems, including RingCentral and the care-management HIPAA compliant platform.
The ability to protect confidential and protected health information (PHI) and consistently follow HIPAA and company privacy requirements is essential.
Work Environment
This is a flexible, primarily independent position involving work in a variety of settings, including:
- Client homes
- Healthcare and residential facilities
- Community locations
- Home office or coworking environments
*You may be at times asked to transport ambulatory clients. (The company carries a non-owned automobile policy)
Normal business hours are Monday–Friday, 8:00 a.m.–5:00 p.m. Work outside these hours occurs only when specifically requested and consistent with the client's service agreement. We are not a 24/7 service.
