The Case Management RN Manager is being sought for a highly awarded western Colorado healthcare organization, offering a strong relocation opportunity for the right leader looking to step into a mountain-town lifestyle surrounded by skiing, hiking, and year-round outdoor adventure. The Case Management RN Manager will guide a highly visible department focused on safe transitions of care and meaningful patient outcomes in a collaborative clinical environment.

The Case Management RN Manager supports a nationally recognized, award-winning health system known for high-tech, high-touch care and consistent recognition including Top 100 Critical Access Hospital status, a 5-Star Patient Experience rating, and Level III Trauma designation. The Case Management RN Manager joins an organization that blends advanced clinical capability with the warmth and friendliness of a close-knit community setting, with strong interdisciplinary support across the continuum of care.

The Case Management RN Manager reports directly to the Chief Nursing Officer and plays a key leadership role within the nursing administration structure. The Case Management RN Manager oversees a small but impactful team of approximately 3 FTEs, providing daily direction while working closely with physicians, social work, utilization review partners, and other department leaders to ensure seamless patient flow and coordination.

The Case Management RN Manager oversees a focused case management department responsible for utilization review, discharge planning, and transitions of care. The Case Management RN Manager supports a tight-knit team that operates in a fast-moving but highly supportive environment, where communication and teamwork are central to success, and where clinicians are deeply engaged in patient-centered decision making.

The Case Management RN Manager is responsible for both clinical leadership and operational oversight, including staff development, competency support, and driving quality improvement initiatives across case management services. The Case Management RN Manager also manages day-to-day operations such as staffing coordination, performance monitoring, regulatory alignment, and collaboration with payers and interdisciplinary teams to optimize patient throughput and resource utilization.
