Full Job Description

Overview
Join our compassionate team as a Personal Care Assistant and make a meaningful difference in the lives of individuals requiring support with daily activities and personal care. In this vital role, you will provide attentive, respectful, and personalized assistance to promote independence, safety, and well-being. Whether working in a home setting, assisted living facility, or group home environment, you will be a trusted caregiver dedicated to enhancing quality of life through compassionate support and professional care.

Duties

Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
Monitor residents’ health by observing vital signs, patient condition, and behavioral changes; report findings accurately
Administer medication and follow care plans in accordance with HIPAA regulations and healthcare standards
Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption as needed
Operate medical equipment such as Hoyer lifts, wheelchairs, and other assistive devices to ensure safe transfers and mobility
Maintain a clean and safe environment through regular cleaning duties and infection control practices
Support clients with cognitive impairments such as dementia or Alzheimer’s disease through specialized memory care techniques
Document all care activities thoroughly in compliance with healthcare protocols and legal requirements
Collaborate with healthcare professionals, social workers, and family members to develop and implement personalized care plans

Experience

Prior experience working in home health, assisted living, nursing homes, or group homes is highly preferred
Demonstrated knowledge of patient observation, vital signs monitoring, medication administration, and HIPAA compliance
Experience caring for individuals with developmental disabilities, autism spectrum disorder (ASD), dementia, or Alzheimer’s is advantageous
Familiarity with patient monitoring tools such as Hoyer lifts and other mobility aids is desirable
Previous roles involving hospice care or long-term care settings will be considered a strong asset
Certification in CPR and First Aid is required; additional certifications such as HIPAA training or nursing assistant credentials are a plus
Ability to perform heavy lifting safely while maintaining proper body mechanics to support residents’ safety
