Nurse Practitioner – In-Clinic & Mobile Wound Care

Wound Care Hawaiʻi is growing, and we’re looking for compassionate, motivated Nurse Practitioners to join our team! We currently have opportunities for both in-clinic and mobile providers on Oʻahu.

About the Opportunity

As a Wound Care Nurse Practitioner, you will assess and treat patients experiencing chronic, surgical, traumatic and difficult-to-heal wounds. You’ll guide each patient through the healing process from the initial evaluation through follow-up care while providing education, encouragement and personalized treatment.

In-Clinic Nurse Practitioners

Provide advanced wound-care services in a consistent, professional clinic environment with access to clinical support, supplies and treatment resources.

Mobile Nurse Practitioners

Bring specialized wound care directly to patients in their homes, assisted-living communities and other care settings throughout Oʻahu. Mobile providers should be comfortable traveling independently and managing a flexible patient schedule.

What You’ll Do

Perform comprehensive wound evaluations and patient assessments
Develop individualized, evidence-based treatment plans
Provide advanced wound-care treatments and clinical interventions
Monitor healing progress and adjust treatment plans as needed
Educate patients, caregivers and families about wound care, prevention, nutrition and healing strategies
Maintain timely, detailed and accurate clinical documentation
Coordinate care with supervising physicians, primary-care providers, specialists, facilities and home-health teams
Communicate changes in patient condition and help ensure continuity of care
Follow all organizational policies, clinical protocols and HIPAA requirements
Build trusted relationships with patients and become an important part of their healing journey

What We’re Looking For

Active and unrestricted Hawaiʻi APRN/NP license
Graduate degree from an accredited Nurse Practitioner program
Current CPR and BLS certifications
Wound-care certification or relevant wound-care experience preferred
Experience in inpatient, post-acute, home-based, long-term, or palliative care is a plus
Strong clinical judgment and assessment skills
Compassionate, dependable and patient-centered approach
Excellent communication, organization, and documentation skills
Ability to work independently while remaining connected to a collaborative care team
Valid driver’s license, reliable transportation, and a clean driving record for mobile positions
