Overview

We are seeking a dynamic and dedicated NHTD/TBI Service Coordinator to lead and manage programs serving individuals with traumatic brain injuries (TBI) and those enrolled in the New York State Home and Community-Based Services (HCB) for the Developmentally Disabled (NHTD) waiver. In this vital role, you will coordinate comprehensive service plans, foster strong relationships with clients, families, and community partners, and ensure the delivery of high-quality social services. Your leadership will empower individuals to achieve greater independence and improve their quality of life through effective program management and team supervision. This paid position offers an exciting opportunity to make a meaningful impact within underserved communities while advancing your expertise in social services administration.

Duties

Develop, implement, and oversee individualized service plans for clients with TBI or developmental disabilities, ensuring compliance with all regulatory requirements.
Manage a team of social service staff, providing supervision, training, and support to promote professional growth and excellent client care.
Cultivate and maintain strong relationships with clients, families, community agencies, healthcare providers, and other stakeholders to facilitate coordinated service delivery.
Monitor program operations, including budgeting, documentation, reporting, and compliance with state and federal regulations.
Lead efforts in program development by identifying community needs, designing new initiatives, and supervising their implementation.
Provide direct client communication to assess needs, address concerns, and ensure satisfaction with services provided.
Supervise staff performance through regular evaluations, coaching sessions, and team meetings to foster a collaborative work environment focused on excellence.

Qualifications

Proven experience working with individuals with developmental disabilities or behavioral health conditions in social services or related fields.
Demonstrated relationship management skills to build trustful partnerships with clients, families, community organizations, and team members.
Strong administrative experience including program development, budgeting, compliance oversight, and report writing.
Proficiency in Microsoft Office Suite (Word, Excel, Outlook) for documentation and communication purposes.
Supervisory experience managing social service program staff or teams within a healthcare or social services setting.
Knowledge of behavioral health principles and experience working with underserved communities is highly desirable.
Bilingual skills are a plus to effectively serve diverse populations; leadership in social services administration is preferred.
Excellent communication skills—both written and verbal—are essential for client interaction and team coordination.
Ability to manage multiple priorities efficiently while maintaining attention to detail in a fast-paced environment.
