Job Overview

We are seeking a highly skilled and dynamic Clinical Manager of Case Management to lead our multidisciplinary team. This pivotal role involves overseeing patient care coordination across various hospital departments, ensuring compliance with healthcare standards, and promoting optimal discharge planning and utilization review. The ideal candidate will possess extensive hospital experience, strong leadership capabilities, and a comprehensive understanding of clinical workflows within complex medical environments.

Duties

Lead and supervise the case management team to deliver exceptional patient-centered care aligned with organizational goals.
Develop and implement care plans that incorporate behavioral health, social work, and behavioral therapy components to support holistic patient recovery.
Oversee discharge planning processes to ensure seamless transitions from hospital to home health or hospice care settings while adhering to regulatory standards such as HIPAA and NCQA standards.
Conduct thorough patient assessments utilizing medical terminology knowledge, physiology, and anatomy expertise to inform care strategies.
Coordinate utilization review activities to optimize resource use while maintaining compliance with Medicare and managed care policies.
Review medical records and documentation for accuracy, completeness, and adherence to hospital policies; facilitate continuous quality improvement initiatives.
Foster leadership within the team by supervising staff performance, providing mentorship, and promoting professional development in areas such as hospital medicine, ICU protocols, emergency medicine, PICU procedures, addiction counseling, and behavioral health.

Requirements

Proven experience in hospital settings with expertise in acute care environments such as Level I Trauma Centers or Level II Trauma Centers.
Strong background in nursing or healthcare management with supervisory experience in case management or social work roles.
Knowledge of hospital operations, including discharge planning, medical records management, documentation review, and utilization review processes.
Familiarity with HIPAA regulations, NCQA standards, Medicare policies, and managed care frameworks.
Excellent understanding of medical terminology, physiology, anatomy, and behavioral health principles relevant to diverse patient populations.
Demonstrated leadership skills with the ability to manage multidisciplinary teams effectively while fostering a collaborative environment.
Experience working within specialized units such as pediatrics (including PICU), emergency medicine, ICU settings, or hospice care is highly desirable.
Strong organizational skills with attention to detail in data collection and patient assessment activities.
