Responsibilities/Job Description:
Fairview Range is hiring a Speech Language Pathologist for our Outpatient Rehabilitation Services department in Hibbing, MN.
In collaboration with patients and families, the Speech Language Pathologist is responsible for providing safe and effective delivery of patient care within scope of practice. This includes examination, evaluation, diagnosis, planning, intervention and establishing outcomes.

Schedule: This position will be working 80 hours per two weeks. The shift length is 8 hours on the day shift. No Weekends.
About Fairview Range Outpatient Rehab:
Fairview Range Rehabilitation Services provides comprehensive adult and pediatric outpatient physical, occupational, and speech therapy services, as well as adult cardiac and pulmonary rehab and athletic training services at our local high school and community college. Our team also provides support to our hospital inpatient and ED departments for rehab-specific evaluations, treatments and recommendations to support appropriate discharge plans and outpatient follow-up.
Our outpatient speech therapy services include:

Cognitive-Communication services to treat attention, executive function, language, memory, problem solving, and post-concussion symptoms.

Dysphagia services and VFSS to diagnose and treat head/neck cancer and swallowing impairments

Pediatric services to treat and support auditory processing disorders, AAC, ASD, developmental delay, expressive language delay/impairment, genetic and chromosomal disorders, pediatric feeding, receptive language delay/impairment, social communication, speech delay/speech sound disorder/apraxia, stuttering/fluency

Speech/Language/Comprehension services to treat and support auditory processing disorders, AAC, aphasia, apraxia/dysarthria-motor speech disorders

Voice services to treat change in vocal quality, chronic cough, and voice disorders

As a Speech Language Pathologist, you will:

The Speech Language Pathologist partners with the patient and family while performing the Evaluation/Re-evaluation

Develops an individualized plan of care through collaboration with the patient and family.

Analyzes data to identify needs and potential risks.

Determines prognosis.
-
Establishes individualized goals with patient and/or caregiver.
-
Individualizes plan of care considering developmental level and cultural differences.

Implements and modifies plan of care based on reassessment and patient response.

Coordinates care and communicate effectively with interdisciplinary team for all aspects of patient care.

Establishes appropriate discharge plan.
-
Implements the plan of care.

Assesses patient and/or caregiver learning needs and provide appropriate education.

Completes documentation and billing accurately and timely

Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served.

Performs all other duties as assigned.

Complies with policies and standards.
Required Qualifications

Master of Science in special language pathology
-
Speech Language Pathologist License in MN, or eligible for Temporary MN License per Clinical Fellowship acceptance.

ASHA-Cert Clinical Comp-SLP
-
Basic Life Support (American Heart Assoc or Red Cross)
