The Pediatric Parenting Connections (PPC) Family Educator works with families in our community with children aged 0-5 who are identified by healthcare providers as needing additional support. The PPC family educator works with families in two different ways. The first (about 75% of the time) is in a pediatric clinic, meeting once or twice with families referred by the pediatricians. The educator provides education and support for breastfeeding, behavior, developmental concerns, school readiness, and positive parenting. The remaining time is spent delivering home visits to families using the Parents as Teachers curriculum, addressing similar topics but in the context of an ongoing relationship. Our Ready for School PPC program is an important component of the community support that Imprints Cares provides to nurture children for lifelong success.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

· Meet one-on-one with families on-site at the pediatric clinic to connect with them and to assess their needs.

· Provide information and resources on parenting skills, positive discipline, child development, breastfeeding, and other developmental topics as applicable.

· Screen children using the Ages and Stages Questionnaires (ASQ-3, ASQ-SE) and other screenings as needed.

· Inform parents of screening results and discuss recommendations regarding their child’s development; refer to early intervention when concerns are found, and follow up on these referrals to support families in overcoming barriers to connecting.

· Responsible for providing timely updates to pediatricians and maintaining open lines of communication with healthcare staff.

· Deliver two home visits a month to the assigned number of families, offering services that meet PAT Model Fidelity.

· Build engaging relationships with families that promote positive interactions and open communication.

· Provide information on parenting skills, typical child development, appropriate discipline strategies, nutrition, and other developmental topics as applicable.

· Administer the ASQ-3 and ASQ-SE and additional screenings, included but not limited to PICCOLO, the PHQ-9, and the Bracken School Readiness Assessment.

· Facilitate early intervention referrals when needed

· Support families in setting goals and in connecting to community resources.

· Maintain detailed timely notes about the services provided, on spreadsheets and/or in the Visit Tracker data system, updating the family’s file after each contact.

· Work with the RFS team to deliver the PAT What You Do Matters class series and additional group connections throughout the year. Classes may be virtual or in-person.

· Work collaboratively with community agencies that address similar concerns to improve community services and resources for families.

· Collaborate with community partners to promote outreach/educational opportunities and

represent Imprints Cares at community meetings and events.

GENERAL:

· Advocates for the mission of Imprints Cares and represents the organization in a professional manner.

· Commitment to professional development, maintaining best practices, and is guided in all work by Imprints Cares’ mission, vision, and core values.

· Team player but also adept at working independently, meeting job specific expectations and timelines.

· Adheres to Imprints Cares’ policies and procedures.

· Able to lift 20 pounds on a regular basis.

· Must have reliable daily transportation for completing home visits; work related mileage is reimbursed at the IRS standard rate.

· Other relevant duties may be assigned as needed.

ATTRIBUTES:

· Adept at leading meaningful conversations; experience with motivational interviewing a plus.

· Comfortable working with diverse populations, respecting the values and cultures of individuals in a non-judgmental way.

· Strong verbal and written communication skills

· Proficient in computer skills and able to use a variety of computer systems.

EDUCATION AND EXPERIENCE:

· Bachelor’s degree in child development, sociology, education, psychology, or a related human services field.

· Fluent in spoken and written English and Spanish.

· At least two years of supervised work experience with young children and/or parents.

· Successful completion of the Parents as Teachers training within 90 days of hire.
