About HCCI
The Home Centered Care Institute (HCCI) is a collaborative non-profit with a mission of increasing access to home-based primary care (HBPC) thereby improving the quality of life for medically complex patients and their families. HCCI is passionately committed to its vision of bringing HBPC into the healthcare mainstream. In addition to being an authoritative source for clinical and operational best practices, HCCI is advancing the field of HBPC through education, consulting, research, and advocacy.

Role Overview
Serves as a key driver of Home Centered Care Institute’s (HCCI’s) revenue growth by ensuring new and existing clients receive a seamless and orderly experience beginning with their onboarding through successful delivery of their contracted products and/or services. Proactively builds a pipeline as well as upsell, renewal, and re-engagement opportunities within an assigned book of business, and owns those opportunities from identification through close. Develops and maintains strong and enduring client relationships, conducts and documents Quarterly Business Reviews (QBRs), and secures renewal or re-engagement of services in line with assigned revenue targets. This person thrives in a fast-paced, target-driven environment, excels at consultative selling and communication, and takes ownership of both client satisfaction and revenue outcomes.

Primary Responsibilities

Sales, Growth & Business Development

Meet or exceed assigned quarterly and annual revenue targets for new inbound and outbound business and renewals, upsells, and client re-engagement.
Proactively prospect and identify growth opportunities within an assigned book of business, building and actively managing a pipeline of new as well as upsell, renewal, and re-engagement opportunities in the CRM.
Respond promptly to inquiries from new and returning clients, providing clear guidance on HCCI products and services with the goal of closing the opportunity – or transitioning it to the Director, Market Growth and Strategic Partnerships – within thirty (30) days.
Identify opportunities to deliver additional value by recommending and upselling HCCI solutions that align with client goals, using a consultative, needs-based selling approach.
Monitor upcoming contract/membership renewals and proactively engage clients to secure timely renewals and maintain continuity of services.
Reconnect with past clients on a structured outreach cadence to understand current needs, gather insights, and collaborate with the Director, Market Growth and Strategic Partnerships to explore opportunities for re-engagement.
Regularly review learner and client evaluations to identify upsell or expansion opportunities, flag risks to other team members, and capture testimonials and ROI data to support the sales narrative.

Client Onboarding & Success

Lead new client onboarding, including establishing early alignment between the client’s goals and expectations and the contracted deliverables.
Ensure a smooth and orderly transition from Sales to the program delivery team, ensuring all stakeholders understand roles, timelines, and goals.
Manage scheduled invoicing throughout the client’s engagement, ensuring timely, accurate billing and resolving any discrepancies in coordination with internal teams.
Monitor engagement progress and proactively address risks, delays, or unmet expectations that could jeopardize renewal or expansion.

Client Retention & Relationship Management

Build and maintain strong, trust‑based relationships with all active clients through consistent, proactive communication and a deep understanding of their goals, challenges, and evolving needs.
Serve as the primary point of contact for client questions, concerns, and strategic guidance, ensuring timely responses and a highly supportive experience.
Monitor client health and engagement throughout the lifecycle of each engagement, identifying risks early and coordinating with internal teams to resolve issues before they impact satisfaction or retention.
Schedule and lead Quarterly Business Reviews (QBRs) with clients at key milestones (i.e. midpoint, post-engagement, or quarterly) to evaluate progress, gather feedback, and identify opportunities for improvement, upselling, and additional support.
Document all QBR notes in the CRM system and liaise with relevant team members to address any successes or concerns.
Advocate for the client internally, ensuring their voice, needs, and expectations are represented in program discussions, planning, and refinements.

General

Maintain timely and accurate records of all client interactions, project status, and feedback in the CRM.
Assist with or assume responsibility for managing special projects, as needed.
Provide recommendations for process development and organizational improvement.
Demonstrate commitment to one’s own professional learning, growth, and development.

Requirements and Qualifications

Bachelor’s degree or equivalent experience in business, healthcare, or related field.
2-5 years of experience in sales, account management, business development, client success, or a related quota-carrying or revenue-facing role; healthcare experience a plus.
Demonstrated ability to identify, pursue, and close upsell, renewal, and new-business opportunities.
Exceptional interpersonal and communication skills, with the ability to build trust and rapport quickly.
Comfort with consultative selling, pipeline management, and working toward revenue targets.
Strong organizational and project management abilities, with attention to detail.
Ability to work independently, prioritize tasks, and manage multiple clients and opportunities simultaneously.
Client-focused and proactive in anticipating needs.
Strong problem-solving skills with a solutions-oriented mindset.
Collaborative, adaptable, and able to thrive in a fast-paced, growth-oriented environment.
Professional, positive, and highly responsive.
Proficient in HubSpot or equivalent CRM system (including pipeline/opportunity tracking), Microsoft Office Suite, and project management tools.
Occasional travel required (<25%).

Compensation is viewed as a combination of base salary, available incentive pay, and benefits.
