Job Summary:

The RN Day Nurse Supervisor at St. Ann’s Home exemplifies the vision, the mission, and the vibrancy of the organization and leads the delivery of exceptional care to our residents. Teamwork and collaboration are instrumental to the success of our communities, and in this role, you will work with the internal leadership team, including our Director of Nursing, and our external partners to oversee and guarantee that our resident's needs are not only met, but delivered with vibrancy. 

Essential Functions:

Day to Day operations: The primary purpose of your job position is develop, coordinate, direct, and administer policies and procedures as to all units within St. Ann’s in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be directed by the Administrator, the Medical Director, and/or the Director of Nursing, to ensure that the highest degree of quality care is maintained at all times.
Duties/Responsibilities: In this role, you will participate in developing, implementing, and periodically updating the resident’s written plan of care, including discharge plan when appropriate. Review physician list of residents seen and new orders obtained to ensure completion of all items addressed. Monitor residents as related to functional declines and report changes in level of function to Physician/Interdisciplinary Team. Routinely run clinical audits to ensure that we are in compliance as directed by the Director of Nursing. Management of medication administration and errors as well as all programs required in a patient’s plan of care such as bowel and bladder programs and laboratory results tracking. Monitoring and following up with any resident and staff incident reports. Responsible for supervision of employees on unit(s).
Administration: Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary. This includes tracking and trending quality metrics for assigned residents and reporting these measures routinely. Review nurses’ notes to ensure that they are informative and descriptive of the nursing care being provided, that they reflect the resident’s response to the care, and that such care is provided in accordance with the resident’s wishes.
Communication: Follow up with family, residents and staff concerns. Perform initial admission chart reviews as needed. Coordination of clinical concerns of residents as they relate to medications, pain, and vital sign abnormalities-report concerns to the Medical Director and Director of Nursing as needed.
Teamwork: Will assume responsibility in the on-call rotation for leadership. Will assist nurses with medication passes and help cover any nurse call-in. Assist with getting coverage for any CNA or nurse call-ins.
Unparalleled Service: Uphold for team and self the highest standards of customer service at all times to residents. Assist team as needed, in order to exceed resident’s expectations.
Flexibility: The ability to shift gears and implement new topics and processes quickly to address emerging resident needs. Work schedule may require conformity to resident needs and outcomes.
Other duties as assigned to meet standards and organizational goals.

Education and Experience Requirements:

Must be a graduate from an accredited approved Registered Nurse Program.
Must possess a current, unencumbered, active license to practice as an RN in this state.
Minimum of two years’ experience in a supervisor role in a hospital, long-term care facility, or other related health care facility.
Minimum of two years’ experience in a long-term care facility.
Current CPR certification required.
Ability to work in a fast-paced environment with multiple competing demands for time and attention.
Ability to lead, direct, and motivate others to innovate and excel.
Must not pose a direct threat to the health or safety of other individuals in the workplace.
Excellent verbal communication skills with the ability to interact with a diverse group of associates, residents, and family members.
Ability to apply reason, logic, and advanced problem-solving skills to resolve complex and/or sensitive issues.
Excellent written communication skills including proper grammar and professional writing.
Ability to read at a level to understand care notes and written directives of medical practitioners.
Strong computer skills and keyboarding skills with proficiency in Microsoft Word and Excel and the ability to quickly master new applications.
Positive attitude, enthusiasm, and energy.

Working Conditions:

Is involved with physicians, residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
Works beyond normal working hours and on weekends and holidays, when necessary, as well as in other positions as needed. Participation in management on-call rotation to fill in on the floor as needed.
Is subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
Is involved in community/civic health matters/projects as appropriate.
Attends and participates in continuing educational programs.
Is subject to injury from falls, burns from equipment, odors, assaults from hostile residents, etc., throughout the workday, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and AIDS and Hepatitis B viruses.
May be subject to the handling of and exposure to hazardous chemicals

Physical Environment:

Must be able to move intermittently throughout the workday.
Must be able to speak and write the English language in an understandable manner.
Must be able to cope with the mental and emotional stress of the position.
Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.
Must function independently, and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
Must meet the general health requirements set forth by the policies of this facility, which include medical and physical examination.
Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times hostile people within St. Ann’s Home.
Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
May be necessary to assist in the evacuation of residents during emergency situations.
