Physical Therapist
Foundation of Resident Care
Job Summary:

St. Ann’s Physical Therapists exemplifies the vision, the mission, and the vibrancy of our organization and contributes to the delivery of exceptional care and rehabilitation to the residents of St. Ann’s Home.
Essential Functions:
Day to Day operations: As a St. Ann’s Home Physical Therapist, you will provide direct patient care; evaluate case history, functional capabilities, limitations and restrictions to develop and deliver an effective rehabilitation plan in order to restore function and minimize pain.

Duties/Responsibilities:

Perform initial evaluation on new patients to assess their current symptoms, establish goals and develop appropriate treatment plans
Develop a plan of care based on physician’s orders and evaluation findings
Continuously evaluates patients' progress and prepares reports that detail progress.
Participates in Care Conferences, Team Meetings and communicates with members of the interdisciplinary team concerning Resident treatment as necessary.
Provides Resident, family, and staff education and training as needed and documents appropriately.
Protects St. Ann’s Residents privacy by keeping information secure and confidential.

Communication: Keep regular, open communication with Director of Rehabilitation Services and onsite Clinical team on any needs of current or predicted changes of condition in regards to St. Ann’s Home Residents

Unparalleled Service: Uphold for self the highest standards of customer service at all times to Residents and employees. Assist team as needed, in order to exceed Residents and Families expectations.

Flexibility: The ability to shift gears and implement new care modalities or plans to fit the evolving needs of the Residents. Creative and out of the box solutions are encouraged and welcomed!

Other duties as assigned to meet standards and organizational goals.

Education and Experience Requirements:

Education / Training: Graduation from an accredited Physical Therapist curriculum
2-5 years of related experience, or equivalent combination of education and experience
Current, unencumbered Physical Therapist licensure in the State of Michigan
Experience working in a long term care setting preferred
Ability to work in a fast-paced environment with multiple competing demands for time and attention.
Ability to be self-motivated to achieve desired organizational and Resident goals.
Must not pose a direct threat to the health or safety of other individuals in the workplace.
Excellent verbal communication skills with the ability to interact with a diverse group of associates, residents, and family members.
Ability to apply reason, logic, and advanced problem-solving skills to resolve complex and/or sensitive issues.
Excellent written communication skills including proper grammar and professional writing.
Strong computer skills and keyboarding skills with proficiency in Microsoft Word and Excel and the ability to quickly master new applications.
Positive attitude, enthusiasm and energy.
Works well in team environment
Manage multiple priorities and deadlines, through disciplined approaches and clear communication.
The ability to locate, track, organize and summarize information.
Able and willing to coach and train others.
Sense of humor!

Working Conditions:

Using hands and arms in handling, installing, positioning, and moving materials, and manipulating things.
Is subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
Performing physical activities that require considerable use of your arms and legs and moving your whole body, such as climbing, lifting, balancing, walking, stooping, and handling of materials.
Frequent exposure to extreme temperatures (Hot and Cold weather).
Is involved in community/civic health matters/projects as appropriate.
Attends and participates in continuing educational programs.
Is subject to injury from falls, burns from equipment, odors, assaults from hostile residents, etc., throughout the work day, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and AIDS and Hepatitis B viruses.
May be subject to the handling of and exposure to hazardous chemicals

Physical Environment:

Must be able to move constantly throughout the workday.
Must be able to speak and write the English language in an understandable manner.
Must be able to cope with the mental and emotional stress of the position.
Must function independently, and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.
Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
May be necessary to assist in the evacuation of residents during emergency situations.

Job Types: Full-time, Part-time

Pay: $45.00 - $48.00 per hour
